Waiver/Release for RISE Performing Arts LLC: I am aware of and appreciate the risks associated with this program, including the risk of catastrophic injury, paralysis, damage, loss or even death. I agree that RISE Performing Arts LLC along with their employees, volunteers, owners, officers and directors shall not be liable for any losses or damages occurring as a result of my own/my child's participation in dance, theatre, music or tumbling/acrobatics program. I further agree not to commence litigation against RISE Performing Arts LLC along with their employees, volunteers, owners, officers and directors or independent contractors relating to its program or any injury or death occurring from my own/my child's participation in it. (As legal guardian), I hereby verify by my signature that I fully understand and accept the above.

Photo Release: The school is hereby granted permission to take photographs of the students to use in brochures, web sites, posters advertisements and other promotional materials the school creates. Permission is also hereby granted for the school to copyright such photographs in its name.
