I hereby certify that the above named child is in good physical condition and is able to participate fully in this program. All current medical and behavioural conditions have been outlined. I understand the inherent risk involved in the physical activity of dancing and I release Roland & Romaine Productions Ltd. and its teachers from liability in case of accident or injury. I understand that all classes will be conducted in the safest possible manner by trained professionals.

I hereby give consent to Roland & Romaine School on Dance to photograph/video my child in their group dance classes, competition and recital. These may be used for training purposes and class instructional videos, as well as with the intent to use images and/or sound recordings on social media and/or promotional purposes.
