I hereby give consent to Roland & Romaine School on Dance to photograph/video my child in their group dance classes, competition and recital. These may be used for training purposes and class instructional videos, as well as with the intent to use images and/or sound recordings on social media and/or promotional purposes.
*You may OPT OUT in your Parent Portal under the "Info - Waivers" dropdown. The studio will be notified of the change, and a note placed on your file.
