Liability Wavier and Release
I, the undersigned, understand that participation in dance classes, rehearsals, performances, and related activates in volves physical movement and carries a risk of injury. I acknowledge that I am voluntarily participating (or my child is participating) in these activities with full knowledge and acceptance of the risk.
I agree to release and hold harmless The Movement Academy, its instructors, staff, volunteers, and affiliates from any and all claims, demands, or causes of action for injuries, damages, or losses that may arise from participation in any and all dance-related activates, including those arising from negligence.
Medical Consent
In the event of a medical emergency, I authorize the staff of The Movement Academy to seek proper medical treatment for myself/my child and to act in their best judgment. I understand that I am responsible for any cost incurred for medical care.
Photo/Video Release
I grant permission for The Movement Academy to photograph and/or record myself/my child during classes, performances, and events. These materials may be used for promotional purposes, including social media, website, print materials, and other marketing channels.
Acknowledgment
I have read and understand this waiver and agree to its terms, I understand that this agreement is binding and that by signing it, I am giving up certain legal rights.
Policies
No refunds once monthly tuition has been paid on the 1st of every month. There is no refund if your child drops a class.
