DANCE STUDIO PARTICIPANT WAIVER AND AGREEMENT

I, the undersigned participant or parent/guardian (if participant is under 18 years old), hereby agree to the terms and conditions set forth by UNITED Performing Arts.

1. **Media Release:**
I grant UNITED Performing Arts permission to use photographs, videos, and other media recordings of the participant for promotional and marketing purposes. This includes, but is not limited to, social media platforms, the studio's website, and promotional materials. If you do not want your child on social media please contact Meg directly.


2. **Respect and Conduct:**
I understand and agree to conduct myself (or my child) with respect towards instructors, staff, fellow participants, and the studio premises. Disruptive or disrespectful behaviour may result in dismissal from classes without a refund.

3. **Zero Tolerance to Bullying:**
UNITED Performing Arts maintains a zero-tolerance policy for bullying or harassment. Participants engaging in such behaviour may face immediate dismissal from classes without a refund.

4. **Uniform Policy:**
I acknowledge and agree that participants are required to wear the designated studio uniform during classes and performances. I understand that adherence to the uniform policy is essential for the overall presentation and unity of the studio.

5. **Fee Payments:**
I understand that dance class fees must be paid in advance for the term. If fees are not paid by the due date I accept a $50 late fee to be added to my term dance fees. Failure to make timely payments may result in the participant's exclusion from classes until fees are settled. I understand that if I am going away or on holidays I will inform the principle of the studio within 3 weeks of the holiday booked and the classes missed will be deducted from your fees. Auto-Pay Agreement: Families who elect to pay through a weekly direct debit or auto-pay arrangement acknowledge that they are committing to the full term enrolment. Should a student choose to discontinue classes during a term, all remaining weekly payments for that term will still be payable. By enrolling and commencing the term, a place has been reserved in the class for the duration of the term, and fees are non-refundable.

6. **Parents' Classroom Access:**
Parents are not allowed inside the classroom during dance sessions to minimise distractions and create a focused learning environment.

7. **Injuries**
I understand that my child is in the hands of qualified and trained teachers, but if in the case of an injury I herby understand that my child is participating at their own risk and in the case of on injury the proper first aid will be put into place and the emergency contact number will be called of the child.

8. **Costumes**
I understand that for the end of year concert my child will have individual costumes for each dance class they are in for the end of year dance concert

9. **Age**
The age group that my child falls into, is the age that my child will be in class with. There is no dancing up an age group or down an age group.


By signing below, I acknowledge that I have read and understood the terms of this waiver and agreement. I voluntarily agree to comply with the studio's policies and guidelines.

2026 PARTICIPANT WAIVER AND AGREEMENT

I, the undersigned participant or parent/guardian (if the participant is under 18 years of age), acknowledge and agree to the following terms and conditions of participation with United Performing Arts.

1. Media Release

I grant United Performing Arts permission to use photographs, videos, and other media recordings of the participant for promotional, educational, and marketing purposes. This includes, but is not limited to, use on social media platforms, the studio website, printed materials, and advertising campaigns.

If you do not wish for your child to appear in photographs or videos, please notify the Studio Principal, Meg Dalrymple, in writing.

2. Respect and Conduct

All students, parents, guardians, and visitors are expected to conduct themselves respectfully towards instructors, staff, fellow students, and studio property at all times.

Disruptive, abusive, or disrespectful behaviour may result in suspension or dismissal from classes without refund.

3. Zero Tolerance for Bullying

United Performing Arts maintains a strict zero-tolerance policy towards bullying, harassment, discrimination, intimidation, or inappropriate behaviour.

Any participant found engaging in such conduct may be removed from classes, performances, competitions, or studio events without refund.

4. Uniform Policy

Students are required to wear the designated United Performing Arts uniform and appropriate dance footwear to all classes, performances, competitions, and studio events.

Failure to comply with the uniform requirements may result in the student being unable to participate in class activities until the correct uniform is worn.

5. Fees and Payments

Dance fees are charged and payable per term and must be paid by the due date stated on invoices.

* A $50 late fee may be applied to overdue accounts.
* Students with outstanding fees may be excluded from classes, performances, competitions, or studio events until accounts are brought up to date.
* Any planned holidays or extended absences must be communicated to the Studio Principal at least three (3) weeks prior to departure. Fee adjustments for approved absences will be considered at management’s discretion.

Auto-Pay Agreement

Families who elect to pay through a weekly direct debit or auto-pay arrangement acknowledge that they are committing to the full term enrolment.

6. Parent Observation and Classroom Access

To maintain a focused and productive learning environment, parents and guardians are generally not permitted inside dance studios during class time unless specifically invited by the instructor or during designated viewing weeks.

7. Injury Waiver and Medical Consent

I acknowledge that participation in dance, acrobatics, cheerleading, aerial arts, and related activities involves inherent physical risks.

I understand that United Performing Arts employs qualified and trained instructors; however, participation is undertaken at the student’s own risk.

In the event of an injury or medical emergency:

* Appropriate first aid will be administered by qualified personnel.
* Every reasonable effort will be made to contact the participant’s emergency contact.
* Emergency medical services may be contacted if deemed necessary.

I release and hold harmless United Performing Arts, its staff, instructors, and representatives from liability for injuries sustained during participation, except where caused by proven negligence.

8. Costumes and Concert Participation

I understand that students participating in the End of Year Concert may require individual costumes for each class they are enrolled in.

Costume fees are separate from tuition fees and must be paid by the specified due dates. Costumes cannot be ordered until payments have been received.

9. Class Placement and Age Groups

Students will be placed into classes based on their age, skill level, and studio requirements as determined by United Performing Arts.

The Studio Principal reserves the right to place students in the class deemed most suitable for their development and safety.

10. Photography and Video Recording by Families

To protect the privacy and safety of all students, parents and guardians must not photograph or record other children without permission during classes, rehearsals, performances, or studio events.

11. Studio Policies and Updates

United Performing Arts reserves the right to amend policies, procedures, schedules, fees, and studio requirements as necessary. Families will be notified of significant changes in writing.
