Injury and Liability Release

I recognize that participation in dance classes, rehearsals, performances, and related activities involves physical movement and the risk of injury. I hereby release and hold harmless Evolution Dance Company–MN, its owners, staff, instructors, employees, and agents from any and all liability, claims, demands, costs, or expenses arising out of or related to any injury, illness, or accident that may occur while participating in any program, class, event, or activity sponsored by or held at Evolution Dance Company–MN. I understand that Evolution Dance Company–MN is not responsible for medical expenses or damages resulting from injury or illness sustained during participation. I agree to assume full financial responsibility for any medical treatment required for my child or myself.

Tuition, Fees, and Withdrawals

I understand that if I choose to drop a class, a 30-day written notice is required to stop tuition charges. I understand that once apparel, costumes, and competition fees are paid, no refunds will be given. If a class or program is canceled due to weather or state mandates, no refunds will be issued.If a student withdraws from a class, no refunds will be given for the months attended.

By signing below, I acknowledge that I have had sufficient opportunity to read this entire document. I have read and understood it, and I agree to be bound by its terms.
I understand that my signature represents my agreement to the policies and waivers stated above.

Media Release
Evolution Dance Company–MN uses photographs and videos of students for publicity, marketing, website, and social media platforms. I grant Evolution Dance Company–MN permission to use photos and videos of my child for these purposes.
