I, hereby release Grace In Motion Dance, Inc. and its staff, volunteers, or members from any and all liability for injuries (including death), illnesses, or loss of property sustained while at Grace In Motion during my/my child’s participation in any and all programming (including off-premises activities or events).
I recognize that activities involving dance could result in physical injury or death. I voluntarily consent to myself/my student(s) participating in dance classes with Grace In Motion. I accept all the associated risks of such activities, and waive any and all claims against Grace In Motion.
In the event of emergency, I hereby consent to allow Grace In Motion, its staff, and volunteers to administer first aid for any injury or illness sustained during participation in dance classes and, if necessary, to seek medical help, including transportation to a medical facility or hospital, or to call an ambulance for the participant(s) mentioned above.
I attest that the participant(s) above is/are in good health and have listed any medical or physical condition that would place them at risk while participating in related activities.
Furthermore, I agree to the following conditions:
â— I understand that under NO circumstance does Grace In Motion issue refunds. All purchases are final in accordance with studio policies and procedures.
â— In the event that a credit is issued for a payment made, said credit will be valid for one year from the date of issue, after which time the credit will expire. Credits are not automatic and must be requested.
â— I give permission to Grace In Motion to photograph/video/interview me/my child and use my/their likeness in publications unless otherwise stated.
GRACE IN MOTION COVID-19 LIABILITY WAIVER
As of September 4, 2023, face masks are OPTIONAL for all dancers, parents, and staff.​
Grace In Motion will always emphasize and prioritize the health and safety of all students and staff. We will collaborate with families, staff, and public health resources on an ongoing basis, to ensure that our studio environment remains healthy, and always in compliance with existing health protocols and recommendations.
If your dancer is not feeling well, is running a fever or is experiencing symptoms of COVID-19 (including, but not limited to coughing, chills, sore throat, headache, shortness of breath, or loss of taste or smell), OR if they have had contact with anyone who has tested positive for COVID-19 in the last two weeks, do NOT send them to the dance studio.
By signing this agreement, I agree that I will not participate in or send my child to any Grace In Motion on-site programming, including studio classes, summer creative arts camps, workshops, etc. if I/my child shows any of the following symptoms of Covid-19. Symptoms may include, but are not limited to:
â— Fever of 99.5 in the last 72 hours
â— Persistent cough
â— Difficulty breathing
â— Chills
â— Muscle pain
â— Sore throat
â— Loss of taste or smell
By signing below I also acknowledge that I have read the foregoing Liability Release Waiver and understand its contents; that I am at least eighteen (18) years old and fully competent to give my consent; That I have been sufficiently informed of the risks involved and give my voluntary consent in signing this Liability Release Waiver as my own free act and deed with full intention to be bound by the same, and free from any inducement or representation.
This waiver will remain effective until laws and mandates relevant to COVID-19 are lifted.
