Medical Waiver
I grant permission to the staff of Sanctuary Dance Academy to take first aid or emergency measures as judged necessary for the care and protection of my child while under the supervision of the studio. In case of a medical emergency, I understand that my child will be transported to an appropriate medical facility by the local emergency unit for treatment if the emergency unit deems it necessary. I understand that in some medical situations the staff will need to contact the emergency resource before the child’s parent, physician, and or other person acting on the parent’s behalf. I also understand and agree that the child’s parents or legal guardians shall be responsible for any expenses incurred.
Liability Waiver
I realize that participation in dance classes and activities could involve some possible personal injury. Despite precautions, accidents and injuries may occur. By signing this release form, I (the dancer age 18 and above or parent/guardian of dancers under the age of 18) assume all risks related to the use of any and all spaces used by Sanctuary Dance Academy. I agree to release and hold harmless Sanctuary Dance Academy, including its owners, teachers, dancers, staff members, and facilities used by both entities from any cause of action, claims, or demands now and in the future. I will not hold Sanctuary Dance Academy liable for any personal injury to myself or my children/children under my care or any personal property damage, which may occur on the premises before, during or after classes. Furthermore, I acknowledge that I have read the studio policies and I agree to obey the studio and facility rules/policies and take full responsibility for mine and my children/children under my care's behavior in addition to any damage I/we may cause to the facilities utilized by Sanctuary Dance Academy.
Media Waiver
I understand that, from time to time, staff members of Sanctuary Dance Academy may photograph and/or video/audio record classes, special events, or recitals, and I grant Sanctuary Dance Academy permission to use any such image or recording of myself or my children for any legitimate promotional purposes.
Studio Policies
FINANCIAL POLICIES
The annual family registration fee and first month's tuition is due upon enrollment.
Tuition is due by the 10th of each month; any tuition received later than this date will be charged a $25 late fee. If payments are made late two times, you will be required to enroll in auto-pay. Please help us meet our financial obligations by paying by the due date. Thank you!
Tuition may be paid via cash, check, credit/debit card or ACH payment (credit/debit card and ACH available online only).
Cash: Please place cash inside an envelope and write the student(s) name on it to ensure the proper account is credited. Please use the exact amount, if possible. Drop the envelope into the tuition drop-box in the lobby.
Check: Please make checks out to "Sanctuary Dance Academy LLC" and place directly into the tuition drop-box (or mail to: Sanctuary Dance Academy, 2305 N. Gateway Avenue, Suite 11, Harriman, Tn. 37748)
Credit Card/ACH Payments* must be made online via the parent portal. Automatic payments may also be set up via the parent portal.
Auto-pay drafts are charged between the 1st and 3rd of each month.
There will be a $35 fee for all returned checks.
All tuition and additional fees (costumes, recital fees, etc.) are non-refundable.
Tuition cannot be refunded due to a student missing class. Students may take a make-up class within one month of the missed class(es) at the director's discretion at a level equal to or just below their own, if available.
Written notice is required when withdrawing from a class by the first day of the month of withdrawal in order to avoid your account being charged.
The class fee is divided into equal monthly payments. Therefore, tuition costs remain the same each month regardless of how many class weeks are in the month.
Families with unpaid balances from a previous year will not be permitted to register for classes until the previous balance has been paid.
Students with a balance on their account will not be permitted to participate in the end-of-year recital unless the balance is paid.
(*Convenience Fees are 3.00% + 0.35¢ for credit/debit cards and 1% + 0.20¢ for ACH payments.)
GENERAL STUDIO POLICIES
Parents and siblings are welcome to wait in the lobby during their student's class time; please ensure children are thoroughly supervised at ALL times and play quietly and respectfully--loud noises are distracting to our dancers in the classroom.
Adults, students and siblings of students are not allowed to use or play in unoccupied studio spaces either before or after class.
No street shoes should be worn inside our studio spaces–this is because we have a specialized dance flooring that is meant for dance shoes only.
Sanctuary Dance Academy follows Roane County Schools for inclement weather closures. Unfortunately, we are no longer able to make up classes missed due to weather closures.
Students that miss an excessive amount of classes in a semester (four or more) may have a smaller part in the spring dance due to the teachers' inability to instruct them in the missed technique training and choreography. A private lesson with the teacher is recommended to help the student catch up. Private lesson rates begin at $60 per hour and vary by teacher.
Required rehearsals must be attended by the dancer in order for them to be allowed to perform in the recital or other performances. No exceptions can be made.
At least one parent is required to volunteer during our spring dress rehearsal. This volunteer position may be either an in-person volunteer time slot or through a volunteer donation (a sign-up list will be sent via email in the spring). While we promise to do our very best to not require any parents to volunteer backstage during the actual recital, we may have a need of parent volunteers if we are short-handed and need more adults to help keep all students properly supervised. Thank you for your participation!
Dress codes are created for a dancer's safety. Students are expected to abide by the dress code while in class.
Dancers must wear cover-ups over their dance clothes when walking in/out of the studio building.
Students must be supervised by their parent or guardian at all times prior to and after class. Students younger than 16 should not exit the building alone or wait outside for pick-up.
Parents, class assistants, or older students must accompany students younger than the age of 5 to the bathroom.
Parents and siblings may watch through the glass doors of the studio but are not allowed to come inside the studio space during class time as this poses a distraction for the students. Thank you for understanding!
Please try to arrive 5-10 minutes before class begins.
A five minute class change-over time is included in the class schedule if needed.
No food or drink is allowed inside the studio rooms except bottled water with a secure lid.
To encourage focus, safety, and social interaction, students are not permitted to use cell phones on the Sanctuary Dance Academy campus without express permission from the teacher or director except to directly contact a parent or guardian when necessary. Phones should be stored away during class. We encourage students to engage with one another and participate fully in the activities provided in lieu of phone usage.
Students and parents must be respectful of teachers, classmates, gym staff, and all other people at all times. Failure to do so may result in dismissal from the studio. Profanity, threatening remarks, or disrespectful speech will not be tolerated.
Sanctuary Dance Academy reserves the right to dismiss a student or family from the studio for any reason at any time without refund of any payments received should it be deemed necessary to do so.
