SCENIC CITY DANCE, LLC
SEASON 16 (2026–2027) ACCIDENT WAIVER, RELEASE OF LIABILITY & ASSUMPTION OF RISK AGREEMENT
By enrolling and/or participating in classes, rehearsals, performances, camps, conventions, private lessons, competitions, workshops, events, or any activities associated with Scenic City Dance, LLC (“SCD”) during the 2026–2027 Season 16 year, the participant and/or parent/guardian acknowledges and agrees to the following:
ASSUMPTION OF RISK
I understand and acknowledge that participation in dance, acrobatics, conditioning, stretching, tumbling, fitness training, rehearsals, performances, camps, conventions, competitions, and related physical activities involves inherent risks of injury. These risks may include, but are not limited to:
Sprains, strains, fractures, and broken bones
Head injuries or concussions
Muscle tears or overuse injuries
Illness, sickness, allergic reactions, or exposure to communicable diseases
Slips, falls, collisions, or accidents
Permanent disability, paralysis, or death
I voluntarily choose to participate and knowingly assume all risks, both known and unknown, associated with participation in any Scenic City Dance activity or event.
MEDICAL ACKNOWLEDGEMENT
I certify that the participant is physically fit and able to safely participate in all activities associated with Scenic City Dance. I further certify that no medical professional has advised against participation and that there are no health-related conditions that would prevent safe involvement in dance activities.
I understand that it is my responsibility to disclose any medical conditions, injuries, allergies, restrictions, or special accommodations prior to participation.
In the event of an emergency, I authorize Scenic City Dance staff, employees, representatives, or emergency medical personnel to secure emergency medical treatment for the participant if deemed necessary. I understand that I am solely responsible for any and all medical expenses incurred.
RELEASE OF LIABILITY
In consideration of participation in Scenic City Dance activities during the 2026–2027 Season 16 year, I, on behalf of myself, my child, my heirs, executors, administrators, successors, and assigns, hereby fully waive, release, discharge, and hold harmless:
Scenic City Dance, LLC, its owners, directors, instructors, choreographers, employees, contractors, assistants, volunteers, staff members, representatives, affiliates, guest faculty, sponsors, venue partners, and event personnel
from any and all claims, demands, causes of action, liabilities, damages, injuries, losses, costs, expenses, or compensation arising out of or related to participation in any Scenic City Dance activity, including but not limited to claims arising from:
Negligence
Premises liability
Equipment malfunction or failure
Accidents or injuries occurring on or off premises
Accidents or injuries occurring from any prop
Travel to or from activities or events
Exposure to illness, viruses, bacteria, pandemics, or communicable diseases
Weather-related incidents, natural disasters, or acts of God
This release applies to all activities conducted by or affiliated with Scenic City Dance during Season 16, whether at the studio, performance venues, competitions, conventions, outdoor locations, or third-party facilities.
PROPERTY LOSS
I understand and agree that Scenic City Dance is not responsible for lost, stolen, or damaged personal property, including but not limited to dancewear, costumes, shoes, electronics, jewelry, bags, or personal belongings brought onto studio premises or to events.
PHOTO, VIDEO & MEDIA RELEASE
I grant Scenic City Dance permission to photograph, videotape, record, and use the participant’s image, likeness, voice, performance, or photographs for promotional, educational, advertising, website, social media, livestream, print, or marketing purposes without compensation or further consent.
CODE OF CONDUCT
I understand that Scenic City Dance reserves the right to dismiss or refuse service to any student or parent displaying inappropriate, disruptive, disrespectful, threatening, or unsafe behavior. No refunds will be issued in the event of dismissal from the program.
INDEMNIFICATION
I agree to indemnify, defend, and hold harmless Scenic City Dance, LLC and all associated parties from and against any and all claims, actions, damages, liabilities, costs, or expenses, including attorney fees, arising from participation in Scenic City Dance activities.
ACKNOWLEDGEMENT
I acknowledge that I have carefully read and fully understand this Accident Waiver, Release of Liability, and Assumption of Risk Agreement. I understand that this document is legally binding and that by signing it, I voluntarily waive certain legal rights.
This agreement shall remain in effect for the entirety of the 2026–2027 Season 16 year and all related Scenic City Dance activities.
PARENT/GUARDIAN CONSENT FOR MINORS
If the participant is under the age of 18, the undersigned parent or legal guardian certifies that they have read and understood this agreement and consent to the minor’s participation in Scenic City Dance activities.
The parent/guardian further agrees to all terms stated above on behalf of the minor participant and agrees to indemnify and hold harmless Scenic City Dance, LLC and its associated parties from any claims brought by or on behalf of the minor.
