Thank you for helping us share what we do in our day to day, class to class at Haven! I will always be conscious in sharing photos that they are appropriate and represent the student and our studio well. I will also ask for students permission before I take any photo or video. -Diandra Photo and Video Release Form for Minors Haven Arts Collective has my permission to use my or my childs photograph and/or class footage publically to promote our studio. I understand the images and recordings may be used in print publications, online publications, presentations, websites, and social media. I also understand that no royalty, fee, or other compensation shall become payable to me by reason of such use.
Policy and Liability Acknowledgement Effective immediately upon execution, I hereby acknowledge that all information provided in registration is accurate and I have read and agree by the Haven Arts Collective Tuition, Policies & Regulations provided in online registration. I understand that tuition will be posted on the 15th of the month prior, and due on the 1st of the month, and that a late fee of $10 per week may be applied to late tuition. Should I choose to unenroll myself or my child from a class, I am bound to tuition payments until written notice is given to Haven Arts Collective. If I unenroll BEFORE the 15th of the month and written notice has been given, I will not be responsible for the following month's tuition. If I unenroll AFTER the 15th of the month regardless of written notice, I will be responsible for the following month's tuition. I understand that any payments made to Haven Arts Collective are non-refundable and non- transferable. I understand that Haven Arts Collective is in no way liable for any injuries sustained in or out of the classroom, while on any studio premises, or on the property of any studio event. I understand that Haven Arts Collective maintains the right to change class placement for any student throughout the year and reserve the right to cancel classes with less than seven attendees. I understand that myself or my child will need to abide by the dress code and expectations stated by Haven Arts collective.
