Thank you for helping us share what we do in our day to day, class to class at Haven! I will always be conscious in sharing photos that they are appropriate and represent the student and our studio well. I will also ask for students permission before I take any photo or video.

Photo and Video Release Form for Minors
Haven Dance Academy has my permission to use my or my child's photograph and/or class footage publicly to promote our studio. I understand the images and recordings may be used in print publications, online publications, presentations, websites, and social media. I also understand that no royalty, fee, or other compensation shall become payable to me by reason of such use.
