2025–2026 Dance Season Contract
Waiver, Release, and Acknowledgement of Policies and Procedures
I have read and agree with the terms of this contract and, for due consideration, agree to engage Morton Street Dance Center for dance instruction and agree with the policies and procedures indicated herein and in the Handbook, which is incorporated herewith.
Sampler Classes
Sampler classes at Morton Street Dance Center are not part of this 10-month contract and are offered as standalone sessions.
10-Month Program Commitment
Once registered, you are responsible for your full accrued and unaccrued tuition for Morton Street Dance Center’s ten-month program, as well as any related registration charges, costumes, or late fees; there are no refunds.
In the event of withdrawal before the completion of the 10-month term, the remainder of your account balance must be paid in full within 30 days. After 30 days, unpaid balances are subject to collections.
Acknowledgement:
☐ I understand that children’s classes are a 10-month commitment and I agree to pay for the full session. I agree to abide by the terms and conditions set forth herein.
Written formal notification is required to withdraw and avoid incurring future fees.
If a dancer’s costume has already been ordered at the time of withdrawal, Morton Street may attempt to cancel the order. If cancellation is not possible, you remain responsible for costume costs incurred.
Registration and Payments
A non-refundable deposit is due at registration to reserve a space in class(es).
There are three (3) registration tiers: VIP, Premium, and Standard, each with different eligibility dates. Early registration improves placement opportunities.
At registration, dancers are responsible for:
First month’s tuition (not handled by autopay),
$65 registration fee (waived for VIP and Premium),
$50 costume deposit per class.
A valid credit or debit card is required on file for all accounts. You may opt to pay via ACH, but a credit card is still required on file.
Auto payments begin on October 1, 2025, and run through April 1, 2026 (or September 1 if on a 9-month plan).
Declined/incomplete payments result in a $40 late fee after 5 days.
Reinstating a declined autopay account incurs a $40 reinstatement fee—no exceptions.
A 3.5% convenience fee applies to each credit card payment (excluding registration).
Acknowledgement:
☐ I understand that I am committing to a full 10-MONTH PROGRAM and will be responsible for all tuition and fees during that term.
☐ I understand that a $50 costume deposit per class is due at registration. Costume balances are due by February 15, 2026. Late payments may affect receipt of costumes.
Account Standing & Late Fees
Accounts with outstanding balances after April 1, 2026, will be assessed a $50 late fee on April 5 and on the 1st of every month thereafter until paid in full.
Costumes and accessories will not be distributed to students with unpaid balances.
Families with balances will also be ineligible to purchase program ad space.
Attendance & Rehearsal Policy
Timely and consistent attendance is essential.
Attendance is taken at the beginning of each class.
Acknowledgement:
☐ A student more than 10 minutes late may be asked to observe class.
☐ Students must be in uniform, with proper shoes and hair up, at least 10 minutes before class. Improper preparation may result in being asked to sit out.
☐ Attendance at all rehearsals before any performance is required.
☐ Missing a final tech or dress rehearsal may result in removal from the performance, at the instructor’s discretion.
☐ Instructors reserve the right to restage choreography or remove students due to three (3) or more absences per semester.
☐ I acknowledge receipt of the 2025–2026 Season Calendar of Events and agree to attend all mandatory rehearsals. This calendar is accessible through my MSDC Account.
2025–2026 COVID-19 and Infectious Disease Protocols
MSDC may continue to require properly fitting masks (minimum 2-layer).
Additional protocols (such as face shields) may be imposed based on public health data.
Only students, faculty, and staff are permitted in the studio.
Students should:
Arrive 10 minutes early,
Enter in dance clothes worn under street clothes,
Remove outside shoes at the door,
Limit personal items.
Class sizes are limited; early registration is encouraged.
Waiver and Release Agreement
In consideration for receiving dance instruction at Morton Street Dance Center, Inc. (MSDC):
I confirm there is no medical condition preventing my child from safely participating in dance.
I understand the risks of injury including, but not limited to: bruises, dislocations, broken bones, or torn/damaged muscles or ligaments.
I understand that these risks apply in person and virtually.
I acknowledge the risk of virus or disease transmission, including COVID-19, and assume all responsibility.
Agreement to Safety Procedures:
My child will stop any activity if experiencing discomfort or symptoms.
My child will follow all instructor directions and safety guidelines.
My child will adhere to virus-prevention behaviors as directed by MSDC and myself.
Indemnification Clause:
To the fullest extent permitted by law, I agree to:
Assume responsibility for any injuries or illness (including virus transmission),
Defend, indemnify, and hold harmless MSDC, its staff, faculty, contractors, and students from all claims, costs, damages, or losses, including attorney fees.
Media Release
Unless I notify MSDC in writing within 14 days of registration, I consent to the unrestricted use of photographs or video taken of my child or myself by or on behalf of MSDC.
Acknowledgement and Agreement

☐ I MAKE THIS AGREEMENT, THESE REPRESENTATIONS, AND CONSENT TO ALL TERMS, COVID-19 PROTOCOLS, AND WAIVERS/RELEASES HEREIN.
