Payment: I understand that tuition will automatically withdraw from my account (card or band draft on file) on the 1st of every month. I understand that if I do not want my card charged for tuition that I must pay via cash or alternate card BY THE 8TH of the month AND alert Mrs. Shannyn so that my card is not double-charged. *Duplicate charges can only be charged if there is a balance on your account. If the balance is $0, no charges can be completed.
I understand that tuition prices can change at any time without notification and that extra charges could arise for various reasons, including but not limited to costumes, accessories, shoes, recital fees, etc.
I agree to pay for all fees that are due for my dancer's tuition by the 1st of every month. I understand that I may ask for an outline of other fees that are due throughout the year and it will be provided to me via email. I agree to pay those fees by the scheduled day as shown in the outline or my dancer's placement or costumes will be affected. If my card/account is charged and returns for insufficient funds, I agree to pay the balance within 5 days from the original attempt or a $10 fee will be placed on my account.

Musical Theater, Conditioning, Contemporary, Company Choreography classes: Dancers are asked to maintain an all-black dress code; leotards, dance shorts, sports bras, crop tops, form-fitting tanks, athletic leggings, and biker shorts are acceptable. Please check the opacity of your dancer's leggings; they are often more see-through than you'd expect! Tights are optional during class, but may assist dancers movement in floorwork and provide some comfort in dance shorts. *Dancers in contemporary class may wear looser-fitting tops/bottoms.
Musical Theater Shoes: Ladies: tan jazz (older dancers may be required to purchase character heels) Gentlemen - black jazz
Contemporary Shoes: Ladies and Gentlemen: barefoot or foot undeez
Company Choreography Class Shoes: Please bring all shoes to class with you each week.
Shooting Stars Program (Parent and Me, Terrific Two's, Broadway Babies, Encore): We want these dancers to be comfortable! These classes may wear any colors. Some great options for little ladies are dance dresses, shorts and a tank top, leotard. Little ladies will need pink tights (a different color of tights will need to be purchased if the dancer is in Christmas show), and pink ballet shoes. Young men will need to wear shorts and t-shirt or tank top and black ballet shoes. Hair should be pulled back. No jewelry. Our youngest dancers should refrain from wearing detachable tutus. They can be very distracting!
Emergencies and Studio Rules: I understand that The Source Performing Arts Studio is not responsible for accidents. I have provided emergency contact information if an accident occurs and will not attempt to hold The Source PAS, its staff, or other students responsible for accidents that occur.
Our studio building is not a tornado shelter. In the event of tornado watch, classes will be cancelled. If students are in the building, dancers will be taken to a safe location until the emergency contact has arrived for pick up. There are shelters nearby should the weather hazards be increasing.
Gum and cups with lids are not allowed in the building. Water is encouraged, as well as healthy snacks. Please clean up after yourself.
I also understand that if my child loses an item, SPAS is not responsible. A lost and found box is kept for the dance year and is donated after recital. We are not responsible for items donated after being in the lost and found for the performance season.
I agree to keep SPAS a drama-free zone! We are family, closely knit and with happy hearts, and want to maintain a positive atmosphere for dancers, parents, and staff.
Band App: I agree to download the Band app for all communication to/from teachers in the studio. I understand that information, closures, news, and reminders will be provided to me on this app.
Publicity Waiver: Unless otherwise informed, SPAS has your permission to use photos/videos of your dancer for publication on the studio's private FB page or on staff member's personal pages, to be used in advertisements in the community, or informational publications.
COVID-19: I understand that my child may be at risk for exposure to COVID-19 by sending him/her to a public space. I understand that SPAS and its staff will continue to implement safety precautions/procedures to maintain the healthiest in-studio setting as possible. Masks are not required. I will not hold SPAS, its staff, or other students in the studio responsible should my child become exposed to COVID-19. Parents are not allowed to wait inside the building during their child's dance class, but may come into the office for payment purposes.
