I am aware of the risks connected with my child participating in gymnastics, tumbling cheerleading and the use of equipment. I agree to not hold accountable or bring legal action against  Strive Athletics LLC, Mccall Connors, Rachael Fuller, volunteers or employees. This waiver releases Strive Athletics LLC, volunteers and employees from all liability relating to injuries that may occur on location before, during, or after activity. As well as any mental or emotional distress that may arise. 

By signing this agreement I agree to hold Strive Athletics LLC entirely free from any liability, including financial responsibility for injuries incurred, regardless of the reasons or circumstances.

I acknowledge the risk involved with this activity including, but not limited to, muscle tears, sprains, and other physical injuries. I certify that my childs participation is voluntary and I have been made aware of the risks. Additionally, my child does not have any conditions that may increase their likelihood of injury. 

I will make every effort to obey the company personnel, all safety rules, and will ask for clarification if needed. 

Additionally, I agree to hold the properties owner and managment company, Best Property Company, completely free from any liability, including financial responsibility for injuries, damage or harm that may occur on the property (877 Main St. Dallas, OR 97338).

Concussion Information & Awareness Form
A concussion is a brain injury and all brain injuries are serious. They are caused by a bump, blow, or jolt to the head, or by a blow to another part of the body with the force transmitted to the head. They can range from mild to severe and can disrupt the way the brain normally works. Even though most concussions are mild, all concussions are potentially serious and may result in complications including prolonged brain damage and death if not recognized and managed properly. In other words, even a “ding” or a bump on the head can be serious. You can’t see a concussion and most sports concussions occur without loss of consciousness. Signs and symptoms of concussion may show up right after the injury or can take hours or days to fully appear. If your child reports any symptoms of concussion, or if you notice the symptoms or signs of concussion yourself, seek medical attention right away.
Symptoms may include one or more of the following:

• Headaches
• “Pressure in head”
• Nausea or vomiting
• Neck pain
• Balance problems or dizziness
• Blurred, double, or fuzzy vision
• Sensitivity to light or noise
• Feeling sluggish or slowed down
• Feeling foggy or groggy
• Drowsiness
• Change in sleep patterns
• Amnesia
• “Don’ t feel right”
• Fatigue or low energy
• Sadness
• Nervousness or anxiety
• Irritability
• More emotional
• Confusion
• Concentration or memory problems
(forgetting game plays)
• Repeating the same question/comment
Signs observed by teammates, parents and coaches include:
• Appears dazed
• V acant facial expression
• Confused about assignment
• Forgets plays
• Is unsure of game, score, or opponent

• Slurred speech

• Can’ t recall events prior to hit

What can happen if my child keeps on playing with a concussion or returns to soon?
Athletes with the signs and symptoms of concussion should be removed from play immediately. Continuing to play with the signs and symptoms of a concussion leaves the young athlete especially vulnerable to greater injury. There is an increased risk of significant damage from a concussion for a period of time after that concussion occurs, particularly if the athlete suffers another concussion before completely recovering from the first one. This can lead to prolonged recovery, or even to severe brain swelling (second impact syndrome) with devastating and even fatal consequences. It is well known that adolescent or teenage athlete will often under report symptoms of injuries. And concussions are no different. As a result, education of administrators, coaches, parents and students is the key for student-athlete’s safety.
If you think your child has suffered a concussion
Any athlete even suspected of suffering a concussion should be removed from the game or practice immediately. No athlete may return to activity after an apparent head injury or concussion, regardless of how mild it seems or how quickly symptoms clear, without medical clearance. Close observation of the athlete should continue for several hours. The new “Zackery Lystedt Law” in Washington now requires the consistent and uniform implementation of long and well-established return to play concussion guidelines that have been recommended for several years:
“a youth athlete who is suspected of sustaining a concussion or head injury in a practice or game shall be removed from competition at that time”
and
“...may not return to play until the athlete is evaluated by a licensed heath care provider trained in the evaluation and management of concussion and received written clearance to return to play from that health care provider”.
You should also inform your child’s coach if you think that your child may have a concussion. Remember, it’s better to miss one game than miss the whole season. When in doubt, the athlete sits out.

Parent or Legal Guardian Signature

If you wish to drop your student from a class, 2 weeks notice is required before the end of the current month. Your child will be able to finish out the current month of classes and will be removed from the roster for the upcoming month. You can email or reach out directly to drop your student from a class.

If your child has been sick please do not have them return to class until they have been AT LEAST 24hrs fever free. If you have any questions about whether your child is well enough to be in class, please reach out. This is to ensure all coaches and other students stay as healthy as possible.

ASSUMPTION OF RISK
I understand that participation in cheerleading, tumbling, gymnastics, dance, and related athletic activities involves inherent risks, including but not limited to slips, falls, collisions, strains, sprains, fractures, paralysis, or other serious injury. I acknowledge that these risks may occur despite careful instruction, supervision, and use of proper equipment.
I knowingly and voluntarily assume all risks, both known and unknown, associated with participation in any classes, practices, open gym sessions, camps, clinics, performances, or events.

RELEASE OF LIABILITY
On behalf of myself and/or my minor child, I hereby release, waive, discharge, and hold harmless Strive Athletics LLC, including but not limited to activities conducted at 760 SE Monmouth Cutoff Road, Dallas, Oregon 97338, and any future locations, off-site events, or affiliated facilities, its owners, directors, officers, employees, contractors, coaches, volunteers, and facility landlords from any and all claims, demands, causes of action, or liability arising out of or related to participation in activities at the facility, including but not limited to claims for personal injury, property damage, or wrongful death, whether caused by negligence or otherwise, to the fullest extent permitted by law.

MEDICAL AUTHORIZATION
I certify that the participant is physically able to participate in athletic activities and has no medical condition that would prevent safe participation, except as noted here:

In the event of an accident or emergency, I authorize Strive Athletics LLC to obtain emergency medical treatment for the participant if I cannot be reached. I understand that I am financially responsible for any medical care provided.

FACILITY RULES & SAFETY
I agree that the participant will follow all gym rules, safety instructions, and coach directions. I understand that failure to follow rules may result in removal from activities without refund.

GOVERNING LAW
This waiver shall be governed and interpreted according to the laws of the State of Oregon.

ACKNOWLEDGMENT & SIGNATURE

I have read this waiver carefully, understand its contents, and sign it voluntarily. I understand that by signing this document, I am giving up certain legal rights.
