The undersigned hereby acknowledges that participation in dance programs and outreach activities involves an inherent risk of physical injury and hereby assumes all such risks. THE DANCE SPACE assumes no responsibility under any circumstances or at any time for personal property of the undersigned. The undersigned hereby agrees that for the sole consideration of THE DANCE SPACE allowing the undersigned to participate and making available to the undersigned for his/her/they use while participating in such programs or activities, certain equipment, facilities, grounds, or personnel, the undersigned participant does hereby waive liability, release and forever discharge THE DANCE SPACE, its instructors and employees from any and all demands, rights, and causes of action of whatever kind or nature, arising out of all known and unknown, foreseen and unforeseen bodily and personal injuries, damage to or loss of property, and the consequences thereof, including death, resulting from my participation in or in any way connected with such dance programs or outreach activities located on the premises or offsite. I acknowledge that it is my responsibility to seek the advice of a medical professional if I have questions about my (my child’s) physical ability to participate in any programming at THE DANCE SPACE. I further covenant and agree that for the sole consideration stated above I will not sue THE DANCE SPACE, its members, instructors, or employees for any claim for damages arising or growing out of my (my child’s) participation in the dance programs or outreach activities.

I acknowledge and consent to allow THE DANCE SPACE to use: photos and/or videos of my child/myself in publications, advertisements, our studio’s website, and/or on social media sites (such as the studio’s facebook or instagram account)

This Contract, Waiver of Liability and Covenant Not to Sue shall remain in effect for as long as I (my child) am (is) a participant in programming at THE DANCE SPACE. I certify that I have read the above carefully before accepting.

The privacy of your personal information is very important to us. We will not sell, rent or trade your personal information to any third party. Special care will be taken to protect the privacy and security of your information. This policy explains our information practices and the way that THE DANCE SPACE collects and uses your information. This Privacy Policy covers the information you provide to us offline and at THE DANCE SPACE website.

We do not collect any personally identifiable information without your consent. We collect and store information from you that you voluntarily choose to give us or enter on our website. When you register for classes at THE DANCE SPACE, we collect contact information such as parent/guardian names, addresses, phone numbers, e-mail address and student information such as names, birthdays, grade, etc. This information is needed to manage studio business and will not be used or sold for any other purpose. We may use the information we collect from you when you register, make a purchase, sign up for our newsletter, etc.

By registering for classes at THE DANCE SPACE, I acknowledge that I am entering into a tuition contract from today until I withdraw from THE DANCE SPACE. I am aware that tuition fees are due and automatically deducted on the 1st of each month. Tuition payments received later than the end of the business day on the 5th of each month will automatically be charged a $15 late fee on their account. An additional charge of 15% of the remaining balance will be accrued for accounts over one month overdue. The account balance must be paid before classes can be taken. Tuition is non-refundable. Additionally, I acknowledge that an annual registration fee of $30 is due.

WITHDRAW POLICY
To withdraw from THE DANCE SPACE, the Membership Withdrawal Form must be filled out by the 20th of the month to avoid the following month's tuition payment. Otherwise, you will be charged for the next month regardless of class attendance. This charge is non-refundable. I understand that I am responsible for paying the monthly tuition if I fail to complete the withdrawal process according to the withdrawal policy. This charge is non-refundable.

MAKE-UP CLASSES
I understand that if my dancer or I cannot attend their regularly scheduled class, THE DANCE SPACE will allow the dancer to make up this class with another class similar in age/level. Make-up classes must be used within the month they were missed. Make-up classes cannot be saved and used during another semester or session. I understand no refunds will be given for any unused makeup classes.

YOUTH PROGRAM PERFORMANCES
I understand that unless opted out via email to allie@chsdancespace.com 7 days before performance package payment due dates (October 1 and February 1), I will be responsible for performance package fees. These fees are non-refundable.

Please note that by submitting, you are providing an electronic mark that is held to the same standard as a legally binding equivalent of a handwritten signature. Your registration will not be completed until payment has been received, and we must have a signed liability waiver on file before the dancer's first class.
