I, the undersigned parent/guardian, hereby acknowledge and agree to the following:
Assumption of Risk: I understand that participation in culinary activities, including but not limited to cooking, baking, food preparation, and handling of kitchen equipment (e.g., knives, ovens, stovetops, blenders), involves inherent risks of injury, including burns, cuts, falls, allergic reactions, and other physical harm. I voluntarily assume all such risks for my child's participation in the House of Junior Chefs Academy programs, whether online or in-person.
Release of Liability: I, for myself, my child, our heirs, executors, administrators, and assigns, hereby release, indemnify, and hold harmless the House of Junior Chefs Academy, its owners, directors, instructors, employees, agents, and volunteers (collectively, "the Academy") from any and all claims, demands, actions, causes of action, losses, damages, costs, and expenses, including legal fees, arising out of or in connection with any injury, illness, death, or property damage sustained by my child or me while participating in any Academy program, whether caused by the negligence of the Academy or otherwise, to the fullest extent permitted by law.
Medical Treatment Consent: In the event of an emergency, and if I cannot be reached, I authorize the Academy to seek and consent to medical attention for my child from any licensed physician, hospital, or medical professional. I agree to be responsible for any and all costs associated with such medical care.
Online Program Disclaimer: For online classes, I understand that the Academy is not responsible for the safety of the environment in which my child participates. I confirm that I will supervise my child during online classes, especially when handling kitchen tools, hot surfaces, and ingredients, and ensure a safe cooking environment.

4. Food Allergy and Dietary Restrictions Policy
Disclosure: It is mandatory for parents/guardians to disclose all known food allergies, dietary restrictions, and medical conditions of their child during the online registration process. Failure to disclose complete and accurate information may result in the child's inability to participate in certain activities or programs.
Academy's Efforts: The Academy will make reasonable efforts to accommodate disclosed allergies and restrictions by modifying recipes or providing alternative ingredients where feasible. However, the Academy operates in a facility where common allergens (e.g., nuts, dairy, gluten, eggs, soy) are present and used.
No Guarantee of Allergen-Free Environment: The House of Junior Chefs Academy cannot guarantee an allergen-free environment. Participants with severe, life-threatening allergies are advised that cross-contamination is always a possibility. Parents/guardians are responsible for assessing the risk and determining if participation is appropriate for their child.
Emergency Medication: Parents/guardians of children with severe allergies requiring emergency medication (e.g., EpiPen) must ensure that the medication is provided to the Academy staff at the start of any in-person program, clearly labeled, and that staff are informed of its location and administration instructions. For online programs, parents/guardians are responsible for having such medication readily available.
5. Code of Conduct
Respect: All participants are expected to show respect towards instructors, fellow students, and Academy property, both in-person and during online interactions.
Safety: Participants must follow all safety instructions provided by instructors, especially concerning the use of kitchen equipment and food handling.

Online Etiquette: For online classes, participants are expected to be attentive, participate respectfully, and maintain appropriate conduct.
6. Photography and Videography Consent
I, the undersigned parent/guardian, hereby grant permission to the House of Junior Chefs Academy to photograph and/or video record my child during program activities for promotional, marketing, and educational purposes (e.g., website, social media, brochures, internal training). My child's name will not be used in connection with these images without my express written consent. If I do not wish my child to be photographed or video recorded, I will notify the Academy in writing at the time of registration.
