I understand that participation in dance classes, rehearsals, performances, camps, workshops, and related events involves inherent risks, including but not limited to physical injury, illness, accidents, or property damage. I hereby knowingly and voluntarily assume all risks associated with participation, whether such risks are known or unknown, foreseeable or unforeseeable.

These risks may arise from, including but not limited to, physical exertion, movement, choreography, instruction, equipment, facilities, travel to and from events, or the actions or negligence of other participants, staff, volunteers, or third parties.

Health & Fitness Acknowledgment
I certify that I and/or my child(ren):
Are physically and mentally fit to participate in dance-related activities
Have no medical, physical, or emotional condition that would prevent safe participation
Have not been advised by a qualified medical professional to refrain from participation
Are in good health and do not pose a health risk to others
I understand it is my responsibility to disclose any relevant medical conditions, allergies, or special needs prior to participation.

Release of Liability
In consideration of being permitted to participate, I hereby waive, release, and discharge Afredanz Dance Center, Afredanz Dance Company, and all divisions thereof, including their owners, officers, instructors, employees, contractors, volunteers, vendors, sponsors, and affiliated facilities, from any and all liability, claims, demands, or causes of action arising out of or related to participation.

This release includes, but is not limited to, liability for:
Personal injury, illness, disability, or death
Property damage, loss, or theft
Pandemics, communicable diseases, or public health emergencies
Accidents, natural disasters, or unforeseen events

This waiver applies whether occurring on-site, off-site, during instruction, performances, events, camps, or while traveling to and from any Afredanz-related activity.

Indemnification
I agree to indemnify, defend, and hold harmless the released parties from any claims, damages, costs, or expenses (including attorney fees) arising out of my or my child(ren)’s participation, whether caused by negligence or otherwise, to the fullest extent permitted by law.

Acknowledgment & Agreement
I acknowledge that this Accident Waiver and Release of Liability:
Is binding upon myself, my child(ren), my heirs, assigns, and legal representatives
Shall be governed and interpreted in accordance with applicable state law
Is intended to be as broad and inclusive as permitted by law

I certify that I have read this document carefully, fully understand its contents, and voluntarily agree to its terms. I understand that by signing this document, I am giving up certain legal rights, including the right to sue.

Company Membership Fees
(Afredanz Dance Company)
Afredanz Dance Company membership includes performance-based training, multiple weekly classes, rehearsals, and company programming. Membership is billed as a flat monthly fee and is not priced per individual class.

Early Pay Discount
A 7% Early Pay Discount is available for payments made by the 2nd of the month
If payment is not received by the early deadline, the discount will be removed

To stop or reschedule an automatic draft, Afredanz must be contacted by the 2nd of the month

Late Payments
Payments made after the 5th are considered late

Tuition fees apply to non-company programs such as Felicia’s U-Nique Steppers and other future classes not part of Afredanz Dance Company.

Agreement Acknowledgment
By enrolling, I acknowledge that I have read, understand, and agree to all Fee Terms & Conditions, including payment schedules, fees, and policies outlined above.

I acknowledge that participation in dance and performing arts activities involves inherent risks, including the possibility of physical injury, illness, or other medical conditions.

By allowing myself and/or my child(ren) to participate in programs, classes, rehearsals, performances, camps, or events hosted by Afredanz Dance Center, Afredanz Dance Company, and all divisions thereof, I knowingly and voluntarily assume all risks associated with such participation.

I understand and agree that:

It is my responsibility to ensure that I and/or my child(ren) are physically and medically able to participate

Any relevant medical conditions, allergies, injuries, or special needs must be disclosed prior to participation

Following all rules, safety guidelines, and instructions provided by instructors and staff is essential for safe participation

I hereby release and hold harmless Afredanz, its owners, instructors, employees, contractors, and volunteers from any liability, claims, or demands arising from participation in dance activities, including injuries or medical conditions that may occur, except in cases of gross negligence or willful misconduct, to the fullest extent permitted by law.

Emergency Medical Authorization

In the event of an accident or medical emergency, I authorize Afredanz staff to obtain emergency medical treatment for myself and/or my child(ren) if I cannot be reached in a timely manner. I understand that I am financially responsible for any medical treatment provided.

I certify that I have read and fully understand this Health Information and Medical Release and agree to its terms voluntarily.

I understand and acknowledge that Afredanz Dance Center, Afredanz Dance Company, and all divisions thereof do not assume responsibility for, nor are they obligated to provide, financial or other assistance in the event of injury, illness, or medical emergency. This includes, but is not limited to, medical, health, dental, or disability insurance coverage or support.

Emergency Medical Authorization

I hereby authorize Afredanz and its representatives to obtain necessary emergency medical or dental treatment for myself and/or my child(ren), including but not limited to first aid, ambulance transportation, hospitalization, or other medical care deemed reasonably necessary for health and safety, if I cannot be reached in a timely manner.

I understand and agree that all medical expenses incurred are my sole responsibility, including any costs not covered by my personal insurance, or in the event I do not have insurance coverage.

Release of Liability

I hereby release and discharge Afredanz, including its owners, instructors, employees, contractors, volunteers, agents, and representatives, from any and all claims, demands, or liability arising out of or related to:

The administration of first aid or emergency medical treatment

Decisions made by Afredanz representatives to seek medical or dental care on my behalf or on behalf of my child(ren)

Any injury, illness, or condition requiring medical attention during participation in Afredanz activities

This release applies to all Afredanz-related classes, rehearsals, performances, camps, events, and travel associated with participation.

Insurance Acknowledgment

I understand that Afredanz does not carry or maintain participant health, medical, dental, or disability insurance. I agree to assume full financial responsibility for any emergency medical or dental costs related to participation, regardless of whether I maintain personal insurance coverage.

I certify that I have read and fully understand this Medical Waiver and Authorization and agree to its terms voluntarily.

hereby grant Afredanz Dance Center, Afredanz Dance Company, and all divisions thereof the irrevocable right and permission to photograph, video record, and otherwise capture images and likenesses of myself and/or my child(ren) during participation in Afredanz classes, rehearsals, performances, camps, events, and related activities.

I authorize Afredanz to use such photographs, video recordings, and other media for lawful purposes, including but not limited to:

Marketing and promotional materials (flyers, brochures, advertisements)

Print and digital publications

Afredanz websites and email communications

Social media platforms (including Facebook, Instagram, TikTok, and similar platforms)

I understand that these images and recordings may include my and/or my child(ren)’s name, likeness, voice, or performance, and I consent to such use without restriction.

I acknowledge and agree that:

No financial compensation or royalties will be provided for the use of these photos or videos

Afredanz retains ownership of all media captured

This consent is granted voluntarily and may be used without further approval

I hereby release and hold harmless Afredanz, its owners, instructors, employees, contractors, volunteers, and representatives from any claims, demands, or liability arising from the use of such photographs or videos, including claims related to privacy, publicity, or defamation, to the fullest extent permitted by law.

This authorization shall remain in effect unless revoked in writing.

I certify that I have read and fully understand this Photo and Video Release and agree to its terms voluntarily.

I understand and agree that I am solely responsible for the care, security, and protection of my personal property while attending or participating in any Afredanz-related activities, including but not limited to classes, rehearsals, workshops, performances, camps, or events.

I hereby release and hold harmless Afredanz Dance Center, Afredanz Dance Company, and all divisions thereof, including their owners, directors, instructors, guest choreographers, employees, contractors, volunteers, event sponsors, and representatives, from any and all liability for loss, theft, or damage to personal property while on Afredanz premises or at any Afredanz-sponsored location or event.

I acknowledge that Afredanz is not responsible for supervising or securing personal belongings and that valuables should not be brought to activities unless necessary.

I further agree to abide by all rules, regulations, and policies established by Afredanz and understand that failure to comply may result in dismissal from activities without refund.

I certify that I have read and fully understand this Protection of Personal Property Waiver and agree to its terms voluntarily.

A non-refundable annual registration fee is required at the time of enrollment for all dancers. This fee is charged at the beginning of each new dance season and must be paid at the same time as the initial tuition payment to secure a dancer’s place in class.

The registration fee covers non-tuition administrative and operational costs, including but not limited to studio insurance, music licensing, communication systems, administrative processing, and other essential studio services. This fee helps Afredanz maintain quality programming while keeping monthly tuition rates as affordable as possible.

A multi-dancer family discount is available for households enrolling more than one dancer in the same season.

For convenience, families may opt into automatic annual drafting of the registration fee at the start of each new season. Enrollment in auto-draft authorizes Afredanz Dance Center to process the registration fee without additional notice at the beginning of each season unless written cancellation is submitted in advance.

Once paid, the registration fee is NON-REFUNDABLE under any circumstances, including withdrawal, schedule changes, injury, relocation, or dismissal.
