By signing below, you agree that you are aware that your child named above will be engaging in a physical activity offered by Triple Arts Academy ("Triple Arts Inc." or "Triple Arts Studios"). Classes offered by Triple Arts Academy ("Triple Arts Inc." or "Triple Arts Studios") are of a physical nature and include the risk of injury either from oneself or another participant or in the use of equipment and facilities, or of illness such as, but not limited to Influenza, MRSA, or COVID-19 that may lead to paralysis or death.

MUSIC PROGRAM

By registering with Triple Arts Academy's Music program(s) , you are committing to either the Full Session (September to February/ February to June). With the exception of 6 & 8-WEEK PROGRAMS, SEMINARS, WORKSHOPS and DAY CAMPS. In these cases, program fees must be paid in full.

SESSION 1: September 09, 2023 to February 11, 2024

SESSION 2: February 12, 2024 to June 15, 2024

MUSIC STUDENTS must commit till the end of the session dates. If withdrawing from classes before the end of the session, the remainder of the full session fees will become due.

KARATE & DANCE PROGRAM

By registering with Triple Arts Academy's group class program(s) , you are committing to either the full season (September 09, 2023 to June 15, 2024). With the exception of 6 & 8-WEEK PROGRAMS, SEMINARS, WORKSHOPS and DAY CAMPS. In these cases, program fees must be paid in full.

1. Triple Arts Inc. requires enrolment in AutoPay for monthly fees, unless paying in full. Payment must be made by credit or debit card Visa/Mastercard and are subject to a 2.75% transaction fee.

2. All class monthly fees are scheduled to be debited on the 15th of the month in the form of AutoPay through DSP Merchant Services.

3. If the full amount of the tuition is not paid on the scheduled day of each month through the AutoPay, a 5% late fee is applied and the total session amount is due immediately.
​
4. If you are changing a GROUP karate and/or dance class membership type for the next month, a 30 days notice MUST be submitted via e-mail before the next scheduled fee withdrawal, otherwise the upcoming monthly fee will be debited.

5. Tuition is non-refundable.

6. If the payment information on file is not available (expired, declined) and no other payment is received, a late fee of $15 will be applied.

7. A $29 fee will be charged for every declined transaction.
​
8. If two consecutive returned declined transactions occur then the remainder of the year's fees MUST be paid in full.

9. Pro-rated classes are available for new students only, who join the class after the beginning of the month.
​
10. Music Lessons will only be credited if administration is given 24 hours notification via e-mail of absence, otherwise student must pay for the missed lesson.

11. Students with unpaid balances will NOT be allowed in class.

Triple Arts Inc. strives to maintain a respectful and positive environment, and we have a zero-tolerance policy for rude behaviour towards our staff, students and families. Any person(s) in violation of this policy will forfeit their registration immediately and will be asked to respectfully vacate the premises.

1. Triple Arts Inc. has the right to combine or cancel group classes if registration for the class is fewer than 3 students.

2. Classes will be cancelled in the case of inclement weather or poor driving conditions. Please use your own discretion regarding attendance if the driving is difficult in your area.

3. If at any time it is required to cancel classes due to an emergency, an e-mail, text or phone call will be placed.

4. If Triple Arts Inc. cancels a class, a make-up date will be announced. No credit will be given for missed classes.

5. Triple Arts Inc. does not operate during statutory holidays.

1. Teachers are ONLY responsible for students during their class time. Parents are responsible for making sure their child arrives on time and are picked up on time.

2. Please do not send students and/or siblings into the studio with a considerable amount of time unattended or if they do not have class.

3. Parents are not permitted in the dojo/studio during class time. Viewing areas are set up for parents.
Instructors will periodically invite parents into the dojo/studio on designated viewing days.

4. If a relative, friend, or neighbor is picking up your child, please inform the instructor ahead of time for safety reasons.

5. Students under 12 years of age must have their parent or guardian present within the premises. *** Exception for Day Camps and Parent's Night Out events.

6. Please try to keep missed classes to a minimum as students will be missing out on valuable training.

7. No refunds will be given for missed classes.

To discontinue or withdraw from classes, we require a 30-day notice prior to the next scheduled fee payment. If notice is not provided, the fee for the following month will be automatically debited. After the payment is processed, registration will then be cancelled.

For example, to withdraw from classes starting on November 18, notice must be given by October 14, before the November 15 scheduled payment.Please note that 4, 6, 8, and 12-week programs, seminars, workshops, and day camps require full payment upfront.

Student and parent understands and accepts these risks and agrees that neither Triple Arts Academy ("Triple Arts Inc." or "Triple Arts Studios"), nor any agent, employee, instructor, nor person associated with Triple Arts Academy ("Triple Arts Inc." or "Triple Arts Studios") will be liable for any injury or damage to the student. The undersigned agrees to indemnify and hold blameless Triple Arts Academy ("Triple Arts Inc." or "Triple Arts Studios"), its directors and instructors, harmless, and releases them from any and all liability for any injury, which may be suffered by the above named individual while on the premises. The registered student also agrees to abide by the policies and rules by the school.

1. Students are asked to be considerate and respectful to all teachers, parents and other students.

2. Only water is allowed in the dojo/studio.

3. Please no jewelry.

4. Please be punctual. A ten-minute grace period will be accepted for latecomers. However, to avoid injuries (lack of warm-up) students later than 10 minutes will be asked to sit and watch the class.

5. Students must wait inside the building for their ride to pick them up.

6. Students must be in proper karate/dance attire for class and hair must be tied back securely.

7. Cell phones: If a student decides to bring their cell phone into the dojo/studio, for security reasons, they will be required to place them in their bag. While classes are in progress, all cell phones must be off or on silent.
