Media and Liability Waiver & Release Form
I, the undersigned parent or legal guardian, acknowledge that participation in dance classes, rehearsals, performances, and related activities involves inherent risks of injury. On behalf of myself and my student, I voluntarily assume all risks associated with participation.
I hereby release, waive, discharge, and hold harmless Cypher Side LLC, doing business as Cypher Side Dance School, its owners, directors, instructors, staff, volunteers, contractors, and affiliated organizations from any and all claims, demands, actions, or causes of action for injury, illness, death, or damage to or loss of property arising out of or related to participation in any Cypher Side activities, whether occurring on or off the premises.
I certify that my student is physically able to participate in dance activities and that I have not withheld any information regarding my student’s physical or medical condition that could affect their safe participation. I understand that Cypher Side Dance School does not provide medical insurance coverage for participants.
This waiver shall be binding upon me, my student, and our respective heirs, representatives, and assigns.
By signing below, I acknowledge that I have read, understand, and voluntarily agree to the terms of this Liability Waiver and Release.
I understand that photographs, recordings, or filming's of students by Cypher Side, instructors, members of the press become property of the Cypher Side may be used for future publicity. I waive any compensation. Photocopies and facsimiles of this release and consent shall have the same legal effect as the original.
