I hereby agree not to hold Mat Valley Dance, its director, and staff responsible for damages or liabilities due to theft, accident, or injury during or resulting from mine/my child’s participation in any capacity or relating to any function or activity of the said Mat Valley Dance. Payments for monthly classes and private lessons are due upon the first week of the month. There will be a 14-day grace period, thereafter a $15.00 late fee per month will be applied to outstanding accounts. 21 days notice is required to withdraw from class and payment obligation. Tuition holds placement in a class, therefor proration for absences and refunds are not issued. Accounts more than 90 days past due will be sent to collections. I hereby assume all financial responsibility for above student enrolled at Mat Valley Dance. In the event it becomes necessary to refer this account for collection, you (as the parent/guardian) will be liable for collection fees. A $50 late fee will be assessed for students not picked up within 30 minutes of studio closure. Students with 3 consecutive absences without notice to administration will be assumed withdrawn from class. I understand Mat Valley Dance follows the same inclement weather policy as the MatSu Borough School District, if schools and/or after school activities close for conditions, Mat Valley Dance closes as well. I understand Mat Valley Dance requires sportsmanlike conduct and agree to be respectful of instruction time and the premises. Students more than 10 minutes late have missed adequate warmup and will not be allowed to participate for reasons of safety.
