Base Dance – Waiver & General Terms
Please read carefully before agreeing
By enrolling at Base Dance Performing Arts (Base-Dance), I understand and accept the following:
⚠️ Risk & Liability
Dance and physical activity carry a risk of injury. I agree that Base Dance, its staff, and venues are not responsible for any injury, illness, or loss that may occur before, during, or after participation. I take full responsibility for my own/my child’s well-being and will notify staff of any medical conditions.
💳 Payments & AutoPay
I understand that fees are charged monthly via AutoPay on the 1st of each month and a 2.75% processing fee applies. By signing up, I give permission for Base Dance to charge my stored card or bank details for tuition and any other unpaid fees (e.g. extra rehearsals or costumes).
📩 Cancelling or Changing Classes
To cancel, change, or leave classes, I agree to give 30 days’ notice by email only to info@base-dance.com.
Verbal notice or messages sent via text or social media won’t be accepted. I remain responsible for any fees due during the notice period.
📆 Changes
Base Dance may occasionally change class times, days, or locations. If this affects your ability to attend, the notice period may be waived.
By continuing with enrolment, I confirm I have read and accept these terms.
