First Aid and Emergency Assistance Consent

I confirm that the student’s medical and emergency contact information is accurate and up to date.

I give permission for trained Base Dance staff to provide appropriate basic first aid and, where necessary, contact emergency services and follow the advice of qualified medical professionals.

I understand that Base Dance staff cannot give legal consent for medical or surgical treatment on behalf of a student. In an emergency, healthcare professionals may provide treatment in the student’s best interests where consent cannot be obtained in time.

For students aged 16 or over, the student may consent to their own medical treatment.
