Liability, Assumption of Risk, Recital Participation Waiver & Photo/Media Release
January 2026 – December 2026 Season

I/we understand that participation in dance classes, rehearsals, performances, competitions, conventions, workshops, and related activities involves physical activity and carries inherent risks of injury. These risks may include, but are not limited to, slips, falls, strains, sprains, collisions with other dancers, use of props or equipment, and physical contact associated with dance instruction and performance.

Despite reasonable precautions taken by The Dance Stop, accidents and injuries may occur. By signing this waiver, I/we (the dancer and parent/guardian, if applicable) knowingly and voluntarily assume all risks associated with participation in any activities conducted by or affiliated with The Dance Stop, including the use of any studio spaces, rehearsal spaces, performance venues, competition and convention venues, and facilities used by The Dance Stop.

Release of Liability & Hold Harmless Agreement

I/we agree to release, waive, discharge, and hold harmless The Dance Stop, its owners, directors, instructors, staff members, dancers, volunteers, contractors, and any facilities or venues used by The Dance Stop (including but not limited to Eastlake North, recital venues, competition venues, and convention locations) from any and all claims, demands, causes of action, or liability arising out of or related to participation, whether occurring before, during, or after classes or events.

I/we agree not to hold The Dance Stop liable for any personal injury, illness, or damage to or loss of personal property that may occur on or off the premises in connection with participation.

I/we further agree to obey all studio, class, and facility rules and to take full responsibility for my/our conduct and behavior, including responsibility for any damage caused to facilities or property used by The Dance Stop.

Medical & Fitness Acknowledgment (No Medical Advice)

I/we acknowledge that instructors and staff of The Dance Stop are not medical professionals and do not provide medical advice, diagnosis, or treatment. I/we affirm that the dancer is physically and medically able to participate in dance activities and has no condition that would prevent safe participation, or that any known conditions, injuries, or limitations have been disclosed to The Dance Stop in writing.
I/we understand that participation may aggravate pre-existing injuries or conditions and assume all risks related thereto. I/we accept full responsibility for any medical expenses incurred as a result of participation.

Illness & Communicable Disease Assumption of Risk

I/we understand that participation in group activities may expose the dancer to communicable illnesses, including but not limited to colds, flu, COVID-19, or other viruses or infections. I/we voluntarily assume this risk and agree to release and hold harmless The Dance Stop from any claims related to illness or disease exposure associated with participation.

Force Majeure / Event Changes

I/we understand that classes, rehearsals, performances, recitals, competitions, or events may be rescheduled, modified, relocated, or canceled due to circumstances beyond The Dance Stop’s control, including but not limited to weather, acts of God, venue issues, government orders, or other unforeseen events. The Dance Stop shall not be held liable for such changes.

Photo, Video & Media Release

I hereby grant The Dance Stop irrevocable permission to photograph, record, and/or capture video of the dancer (the “Media”) during classes, rehearsals, performances, competitions, workshops, or any studio-related events.
I authorize The Dance Stop to use, reproduce, edit, crop, alter, compile, publish, and distribute the Media, in whole or in part, for print, online, social media, video, advertising, promotional, and marketing purposes, as well as other studio publications, without limitation as to time or geographic location.

I understand and agree that:
There is no expectation of privacy or confidentiality related to the Media
No financial compensation, royalties, or ownership rights will be provided now or in the future
Permission granted is irrevocable and cannot be withdrawn retroactively
I hereby release and hold harmless The Dance Stop, its employees, contractors, and any third parties involved in the creation or publication of the Media from any claims arising from the use of the Media.

Photo/Media Opt-Out
Families who wish to opt out of photo and media usage must complete the Photo Opt-Out Waiver and check the appropriate box. The Dance Stop will make reasonable efforts to honor opt-out requests; however, opt-out may not apply to group photos or videos where individual removal is impractical.

Acknowledgment & Signature
By signing below, I/we acknowledge that I/we have read, understand, and voluntarily agree to the terms of this waiver and release. If the dancer is a minor, the parent or legal guardian agrees to these terms on the minor’s behalf and accepts full responsibility for the minor’s participation.

Recital Participation & Liability Waiver

I/we understand that participation in The Dance Stop recital, dress rehearsals, and related recital activities involves physical activity and performance in a live venue setting, and that there is a possibility of personal injury. Despite reasonable precautions taken by The Dance Stop, accidents and injuries may occur.

By signing this release form, I/we (the dancer and parent/legal guardian, if applicable) knowingly and voluntarily assume all risks related to participation and the use of any and all spaces used by The Dance Stop, its instructors, staff, volunteers, and facilities.

Release of Liability & Hold Harmless

I/we agree to release, waive, and hold harmless The Dance Stop, including its owners, teachers, instructors, dancers, staff members, volunteers, and any facilities or venues used by The Dance Stop, including Lisa Stanziale and Eastlake North High School, from any and all causes of action, claims, or demands now or in the future.

I/we agree not to hold the above parties liable for any personal injury, illness, or damage to or loss of personal property that may occur on the premises before, during, or after dress rehearsals and recital performances.

I/we further agree to obey all class, backstage, and facility rules and accept full responsibility for my/our conduct and for any damage I/we may cause to facilities utilized by The Dance Stop.

Costumes, Props & Personal Belongings

I/we acknowledge that costumes, props, accessories, footwear, and personal belongings are brought to recital venues at my/our own risk. The Dance Stop is not responsible for loss, damage, theft, or misplacement of any personal property.

Assumption of Risk – Venue & Performance Environment

I/we acknowledge that recital venues present additional risks beyond regular classes, including but not limited to:
Stairs, risers, and backstage areas
Lighting, sound equipment, cables, and stage elements
Crowded dressing rooms and performance areas
Unfamiliar flooring and performance surfaces

I/we voluntarily assume all risks associated with participation in recital-related activities.

Medical Emergency Acknowledgment
In the event of an injury or medical emergency during recital activities, I/we authorize The Dance Stop staff to seek emergency medical treatment for the dancer if necessary. I/we understand that I/we are solely responsible for any medical expenses incurred.

Acknowledgment
