I choose to participate in classes, performances, workshops, and other activities at Ballet² Dance Center of my own free will and certify that I am in proper physical condition to take part in such activities. If I have questions about whether an activity is suitable for me to pursue, I will consult my health care provider in making that decision. If I have any known physical vulnerabilities, conditions, or injuries, I agree to discuss them with the director before participating.

Release
By accepting this document, I release Ballet² and its directors, owners, students, teachers, staff, employees, volunteers, associates (collectively referred to in this document as Ballet²) from any liability or claim that I or my representatives may have against Ballet² with respect to any bodily injury, personal injury, illness, death, or property loss or damage that may result from my participation at Ballet².
I voluntarily release and forever discharge and hold harmless Ballet² from any and all claims or demands for damages, loss of services, costs and expenses, injuries, attorney fees, and any other call for reparation from any and all injury to me or my property arising in any way from my participation in dance classes, camps, intensives, workshops, performances, troupes, the use of Ballet² equipment or facilities, and any activities associated with Ballet².
Risks
I understand that there are risks of physical injury associated with, arising out of, and inherent to dancing. These risks include the potential for slips and falls, sprains, strains, dislocations, soft tissue injuries, musculoskeletal injuries, podiatric conditions, and other risks not specified here. Understanding these risks and the potential for others not listed, I agree to personally accept and assume all of the risks present in my participation at Ballet² My participation at Ballet² is entirely voluntary, and I choose to participate in spite of the risks.

Dance education sometimes requires hands-on instruction as well as verbal instruction. Instructors may correct dancers by touching their arms, legs, feet, hips, back and head to move them in the correct position. I acknowledge that this is a common standard in dance instruction and understand that it is my responsibility to communicate clearly with my teacher and/or the director if any form of touch is unacceptable to me.

Medical Treatment and Insurance
I understand that Ballet² does not assume any responsibility for or obligation to provide financial or other assistance in the event of injury or illness, including but not limited to medical, health, or disability insurance or support.
I authorize Ballet² to obtain necessary medical or dental treatment, including first aid, ambulance transport, hospitalization, or such other care necessary for my health and welfare in an emergency. If my insurance does not cover emergency treatment that is deemed necessary and sought for me by Ballet², I agree to be responsible for and pay all costs incurred on my behalf.

I release and discharge Ballet² from any claim which may arise on account of any first aid, treatment, or service rendered in connection with my participation in Ballet² activities or with the decision by any representative or agent of Ballet² to consent to medical or dental treatment on my behalf in an emergency.

I understand that Ballet² does not carry or maintain health, medical, dental, or disability insurance coverage for any participant. I agree to take responsibility for full payment of any emergency medical or dental costs related to my Ballet² participation regardless of whether I have insurance coverage.

Photographic Release
I understand that Ballet² may take photo and video recordings of me during my participation in Ballet² classes and activities. I convey to Ballet² full rights and interest in these recordings. I understand such recordings may be used in advertising or other published materials, physical or virtual.

If I do NOT consent to being photographed or video-recorded, I will make sure the director is aware of my concerns and the reasons for them, I will be proactive about avoiding being photographed or recorded, and I will hold Ballet² harmless if a photo or video recording of me is released despite all precautions. I understand that this choice may limit my participation in performances that are routinely photographed and/or videotaped.

BALLET SQUARED
POLICIES, PROCEDURES and WAIVERS
By registering for any class, session or program at Ballet Squared, all students and parents/guardians indicate their awareness, acceptance and consent of all studio Policies, Procedures and Waivers as detailed on this document, which may be supplemented or amended from time to time. In the event of a mid-session supplementation or amendment of the within Policies, Procedures and Waivers, notification will be sent to all active accounts by email.

TUITION PAYMENT POLICIES -AUGUST THROUGH JUNE
A Non-Refundable $25 REGISTRATION FEE per student, per year is due at the time of registration.
TWO PAYMENT OPTIONS:
OPTION ONE: Monthly Installment – 11 installments – August through June.
OPTION TWO: Semester Installment with 2 installments due on August 1st and January 1st.
Ballet Squared's holidays and school closings will be noted within the August to June calendar. Refer to SCHOOL CALENDAR for all school closings planned in advance. Other school closings may arise due to circumstances out of our control (snow days, coronavirus, loss of electricity, etc). In those cases, all students will receive email/text notification and announcements will be on our facebook page. Class Make-Up Policy will go into effect or ZOOM classes will pick up, whichever is deemed appropriate. However, no tuition will be refunded, transferred, or discounted. There is a separate recital fee and show fee for RECITAL AND SHOW PARTICIPANTS ONLY. There will be costume fees for each performance as well. The fees and due dates will be announced by director. Regular schedule of classes will continue until the end of May. After that date, only recital rehearsals will continue with class parties scheduled for the week after recital. All Monthly Installment and Semester Installment tuition payments are NON-REFUNDABLE and NON-TRANSFERABLE. Dance lessons are a commitment from both the parents and students. No monies will be refunded, held on account or transferred to another student no matter what the circumstance or reason. Families must be sure of their schedule as our NO REFUND POLICY is strictly enforced. Ballet Squared reserves the right to transfer any class or program to ZOOM instruction if they see fit for reasons beyond its control, including, but not limited to, concerns or restrictions relating to the coronavirus. In the event of any such transfer, no monies will be refunded and no discounts or transfers of tuition will be given. By registering for classes at Ballet Squared, families agree to provide a designated area for their child(ren) to take classes remotely from their home in the event that the decision is made to transfer any class or program to ZOOM instruction. In addition, families understand and agree to accept the modified schedule via ZOOM if this situation arises. The modified ZOOM schedule will be well thought out in the best interest of the students and faculty. Ballet Squared reserves the right to shorten the duration of classes and/or otherwise modify the class schedule for ZOOM instruction as it sees fit. Again, no refunds will be given for classes that are transferred to ZOOM instruction.

Ballet Squared accepts PAYPAL, CASH, CHECK, VISA, MASTERCARD, DISCOVER, and DEBIT CARDS for all payments. Payments can be made through DanceStudio-Pro or in person.
Late Fee/Returned Check Policy
A $15 Late Fee will be added monthly to any account if tuition is not paid before the 10th of the month – this applies to both Monthly and Semester Installment payments. A $35 Return Check Fee will be charged for any check returned by the bank for any reason.

Add/Drop Classes
If a student and/or parent desires to add/drop a class/classes, the director must be notified via email with detailed reasons as to the change. A meeting with the director may be scheduled to discuss these changes.

Missed Classes/Make-up Classes
If parents/guardians so choose, a student may attend a make-up class in the event they miss a class due to illness, outside appointments, inclement weather, or circumstances beyond our control such as, but not limited to, coronavirus, power outage, etc. Make-up classes may be taken in any other same-level class or a level below the student's current level. No formal notification of make-up classes is necessary. It is the responsibility of each family to keep track of classes missed if they so choose. Make-up classes may be taken before or after the missed class. No deductions in Monthly or Semester Tuition Installments will be made for classes missed for any reason.

Performance Participation Policy
Students may participate in the Student Productions ONLY if accounts are current. This includes tuition, late fees and all costs associated with the Student Performance. Holiday Show will be held in December, exact date TBD. Recital will be held in June, exact date TBD. Dress Rehearsals must be attended in order to participate in the performances. Students put much effort into preparing for these performances with original choreography and we want both the student and the parents/caregivers to enjoy and celebrate each student's accomplishments.

Photography/Image
Ballet Squared may take images (photographs and/or videos) of enrolled students and families that may be used to document activities of Ballet Squared. These images may be used in promotional materials, social media accounts and websites of Ballet Squared. All images are for the exclusive use of Ballet Squared and are property of same. If any parent or guardian does not want their or their child's image used, they must notify and inform the office via email at the time of registration.

Waiver of Liability and Medical Release
All enrolled students and their parents/guardians consent to the registrant’s participation in Ballet Squared's physical programs, classes, activities and performances, and acknowledge that they are aware of the significant risks and possibility of physical injury associated with participation in such programs, activities and performances. Enrolled students and their parents/guardians recognize and willingly assume such risks. Further, by registering for any class or program at Ballet Squared, parents/guardians represent that registrants do not have any illness, disability or impairment that will endanger the registrant or others in connection with the registrant’s participation in the dance programs, classes, activities and performances offered by Ballet Squared, Parents/guardians represent that all students enrolled are in good physical condition and do not suffer from any illness, disability or impairment that would prevent or limit participation in any enrolled program, class or activity. In consideration for Ballet Squared's accepting registrants for its programs, classes, activities and performances, all registrants and parents/guardians release, discharge, indemnify and hold harmless Ballet Squared and its owner, employees, teachers and associated personnel from and against any and all claims by or on behalf of the registrant as a result of the registrant’s participation in such programs, classes, activities and performances. All registrants and parents/guardians waive the right to bring any claim or suit and voluntarily release and discharge Ballet Squared, its owner, employees, independent contractors and agents from any and all claims, demands, causes of action, liability, damages, costs or expenses arising out of or relating to a registrant’s participation in Ballet Squared's programs, classes, activities or performances. Parents/guardians authorize Ballet Squared through its staff and personnel, to obtain, consent to and authorize emergency medical care or treatment in the event that such care is necessary or appropriate in the opinion of the staff and personnel for the benefit of a student due to accident, injury or illness. Ballet Squared will only exercise such consent and authorization for medical care or treatment in the absence of a student’s parent/guardian and only if it is unable to reach a student’s parent/guardian within a reasonable period of time given the circumstances of the emergency. Parents/guardians will be responsible for making payment of all costs of any such medial care or treatment directly to the hospital, physician or facility providing such care.
Drop Off and Pick Up Policy
Advanced students may not arrive earlier than 30 minutes prior to their scheduled class time. All other students may not arrive earlier than 10 minutes prior to their scheduled class time. The studio is not responsible for any student who is dropped off early or not picked up in a timely manner. Please be considerate: if your child attends the last class of the evening, please arrive early to pick up your child so the studio may close as scheduled. Students must be picked up on time. If an emergency occurs hindering timely pick up, please notify the office. After l0 minutes past the end of scheduled class, a $20 fee will be charged for every fifteen minutes that a student is not picked up. This fee will be added to the following month's tuition. Again, the studio is NOT responsible for students who are not picked up on time. If your child is under the age of 12, please walk them into the building and come back to the door to pick them up.
Grounds For Dismissal
Every student is expected to behave in a disciplined, responsible and courteous manner when on studio grounds, and when participating in studio activities and performances, as well as dress appropriately and neatly for class. Any use of drugs, alcohol or tobacco products will not be tolerated on studio grounds. The Director reserves the right to suspend or expel any student at any time if his or her attitude, conduct, language, appearance, attendance, payment record or the like is judged unsatisfactory. Any consistently disruptive or inappropriate conduct or behavior will be grounds for dismissal at the discretion of the Director. In the event of a dismissal, all tuition and/or recital monies paid to Ballet Squared on behalf of a discharged student will be forfeited. Parents/Guardians also must respect the rules, policies and procedures of Ballet Squared and abide by driving slowly and cautiously around the building, and refraining from any rude or aggressive behavior toward any other Ballet Squared student, family member or staff member.
Any and all incidents should be reported immediately to the Director.

Choreography
All choreography is original. It is protected under copyright law. Copyright infringement including a legal claim will occur if choreography is used without legal authorization (publicly performed, reproduced, distributed or adapted).
