DIVINE INSPIRATIONS CENTER FOR THE ARTS (DICA) / DIVINE DYNASTY
PARTICIPATION WAIVER & PARENT ACKNOWLEDGMENT
By registering my child with Divine Inspirations Center for the Arts (DICA)/Divine Dynasty, I acknowledge and agree to the following:
PARTICIPATION & ASSUMPTION OF RISK
I understand that dance, majorette, conditioning, tumbling, rehearsals, performances, competitions, parades, outdoor training, and related activities involve physical activity and inherent risks, including falls, collisions, strains, sprains, and other injuries. I voluntarily authorize my child to participate and assume the ordinary risks associated with these activities.
RELEASE OF LIABILITY
To the extent permitted by law, I release and hold harmless Divine Inspirations, Inc., Divine Dynasty, its directors, employees, coaches, instructors, volunteers, contractors, partner facilities, and authorized representatives from claims arising from the ordinary risks of my child's participation. This release does not waive rights or claims that cannot legally be waived.
MEDICAL & EMERGENCY AUTHORIZATION
I certify that I have disclosed relevant medical conditions, allergies, medications, injuries, or physical limitations that may affect my child's safe participation. If I cannot be reached during an emergency, I authorize DICA/Divine Dynasty staff to obtain reasonable emergency medical assistance for my child. I understand that I am responsible for expenses associated with such care.
ATTENDANCE & PARTICIPATION
I understand that consistent attendance, punctuality, preparation, conduct, effort, and coachability are important components of the Divine Dynasty program. Excessive absences, tardiness, missed required rehearsals, or failure to meet team expectations may affect choreography assignments, performances, competitions, leadership opportunities, or team placement.
TUITION & FINANCIAL RESPONSIBILITY
I understand that registration fees, tuition, costumes, uniforms, competition expenses, team gear, and other applicable charges are my financial responsibility according to the policies and deadlines communicated by DICA.
Monthly tuition is due on the 1st of each month and no later than the 5th to avoid applicable late fees. I understand that my account must remain current and that approved payment arrangements must be requested in advance.
DICA PROPERTY
I understand that uniforms, costumes, accessories, equipment, or other items identified as DICA-owned or loaned property must be returned when requested and in reasonable condition. I understand that my account may be charged for lost, damaged, or unreturned property as permitted by DICA policy.
PHOTO, VIDEO & MEDIA RELEASE
I understand that photographs and video recordings may be taken during DICA/Divine Dynasty activities. I authorize DICA/Divine Dynasty to use my child's image, likeness, and performance footage for legitimate organizational purposes, including social media, websites, promotional materials, community outreach, grant reporting, and program documentation, without additional compensation.
COMMUNICATION & POLICIES
I understand that BAND is Divine Dynasty's primary platform for team communication and that I am responsible for reviewing program communications, schedules, deadlines, and announcements.
I acknowledge that participation in DICA/Divine Dynasty is subject to the organization's current policies, procedures, codes of conduct, and program requirements as communicated to families.
PARENT/GUARDIAN ACKNOWLEDGMENT
By completing registration and electronically accepting these terms, I certify that I am the parent/legal guardian or otherwise authorized to enroll the participating child. I acknowledge that I have read, understand, and agree to these terms and accept responsibility for complying with DICA/Divine Dynasty policies throughout the season.
