MEDIA RELEASE
I give my permission for photographs, video or television footage, which includes my child/children to be used for promotional purposes in television, newspaper, magazines, social media, or any other media.

MEDICAL WAIVER
As the parent/legal guardian for my child, I understand the sport of acro, gymnastics, ballet, any dance genre, cheerleading involves many risks of injury. In consideration for INSPIRING DREAMS STUDIO permitting my child to participate in the program, I hereby agree to hold INSPIRING DREAMS STUDIO, its employees or agents harmless from any and all liability which may arise from participation in any activities of the program. These terms serve as a release. I approve administration of minor first aid to be performed by INSPIRING DREAMS STUDIO staff. In the event of a medical emergency, INSPIRING DREAMS STUDIO will make every effort to contact you immediately. If we cannot, INSPIRING DREAMS STUDIO will contact 911 for medical assistance or transport your child to Page Memorial Hospital. By acknowledging this waiver, you hereby grant permission for medical facilities and physicians to administer care and treatment and agree to pay all debts associated with such treatment.

MINOR CONSENT/ASSUMPTION OF RISK
I am aware that participation in the sport of gymnastics, ballet, any dance genre, cheerleading can be dangerous activity involving many RISKS OF INJURY. I understand the dangers and risks of participation, which can include, but not limited to death, serious neck or spinal injury which can result in paralysis, brain damage, serious injury to all internal organs, injury to all bones, ligaments, muscles, tendons, and other aspects of the body. I understand the dangers and risks may also result in impairment of future abilities. I understand that because of the dangers of these sports, it is important to follow the instructor's directions regarding techniques, training, and other rules and agree to obey all instruction. I hereby assume all of the risks for my child associated with the sports of gymnastics, ballet, any dance genre, cheerleading and any and all classes and agree to hold INSPIRING DREAMS STUDIO, and its employees or agents harmless from any and all liability, cause of action, debt claims, or demands of any nature whatsoever which may arise in connection with the participation in any activities related to the program or studio. The terms hereof serve as a release and assumption of risk for my heirs, estate and for all members of my family.

SEXUAL HARASSMENT
All employees have been trained in the proper conduct in regard to avoiding sexual harassment. Any such harassment will not be tolerated from employees, nor from clients of Inspiring Dreams Studio.

PAYMENT POLICY
Payments are due the FIRST of each month. If payment is not received by the 10th of the month, you will be assessed a LATE FEE of $15.00. The program season runs from September through May. If your child stops attending, you are still responsible for payments for the remainder of the season. Once classes fill, we do not except students throughout the season to ensure class/student progression.
There is a $5.00 registration per student charged at the time of registration.

2025/2026 I ask that you sign up for the auto-payment for tuition. A service fee of 2.75% will be charged each month for credit/debit card payment and for ACH Check payments.

STUDIO POLICIES
INSPIRING DREAMS STUDIO reserves the right to cancel a student's membership in the program at any time without cause or explanation. We also reserve the right to schedule additional classes and add other studio activities at any time or change/cancel classes as deemed necessary without warning or explanation. In the event of cancellation, we will offer a make-up time. Instructors will notify you of their make-up class.

CLOSED CLASSES - At all ages, students can get very distracted by parents or siblings. Please arrive at the front entrance of the studio from main street no earlier than 5 minutes prior to your class start time to allow the instructors to finish their prior class without interruption. Instructors will get their students at the front door.

CLASS CANCELLATIONS/INCLEMENT WEATHER - Parents will be notified through parent portal by text or email and on Facebook. If a class is cancelled it will be by 1pm. If you have not received a message, classes are still on as scheduled. Make up classes will be scheduled, whenever possible, for all days missed due to inclement weather. Please do not contact the instructor about weather cancellations. Again, if we cancel will send out bulk messages prior 1pm.

PICK-UP - A student is not allowed to leave the studio without a parent or guardian. Please be prompt when picking up your child.

PARKING - Please do NOT park in the parking lot in the back of the studio. Parking will be available on Main Street, behind the old Brown's restaurant building.

COSTUMES/LEOTARDS - Parents are responsible for costume and leotard fees. Payment should be made promptly upon receiving your notice. Students will not receive their costume/leotard until payment has been made. All costume/leotard payments must be made directly to their instructor, not the studio.

PERSONAL BELONGINGS - Valuable items should NOT be brought to the studio. INSPIRING DREAMS STUDIO is NOT responsible for lost or stolen items. Water is allowed during classes; however, NO FOOD should be brought to class.

FUNDRAISERS - During the season, we may have studio fundraisers to help offset any and all costs of events, competitions, equipment or other activities or trips. We ask that all students of the studio help participate in these fundraisers.

Please be respectful towards the instructors and the studio owner and limit contact outside of classes. They all have day jobs and families. If you have an emergency and need to contact the instructor, go to the contact section of the portal and scroll down to the instructor you need.

****I acknowledge I am responsible for monthly tuition from September through May for any and all classes enrolled and that I have read the waiver/policy thoroughly and am compliant with it.
