2025–2026 Parent Waivers
PARTICIPATION WAIVER
As the legal parent/guardian, I release and hold harmless Zeal For Life Dance Company, its owners, operators and teachers from liability related to loss, damage, or injury—including death—that may occur during participation.
MEDICAL EMERGENCIES
In the event of an emergency where a parent cannot be reached, ZFLDC has my permission to seek medical treatment for my child. I certify that my child is in good mental and physical health.
TUITION & PAYMENT POLICIES
Registration Fee: $55 per student (non-refundable)
Monthly Tuition: Auto-drafted on the 1st of each month
Late Fees: $20 for returned payments and/or payments that fail to process
WITHDRAWAL POLICY
A written 30-day notice via email (zfldancecompany@gmail.com) is required. Failure to give notice will result in the next month’s tuition being charged. No refunds or prorating for mid-month withdrawals.
CANCELLATION & SUBSTITUTIONS
If ZFLDC cancels classes due to power outages, weather, or emergencies, classes may be held virtually. No refunds provided.
Substitute instructors may teach in the event of staff absence. If no substitute is available, a make-up class will be scheduled.

Nov. 11 – Veterans Day
Nov. 26–30 – Thanksgiving Break
Dec. 22–Jan. 4 – Winter Break
Jan. 19 – MLK Day
Apr. 3–12 – Spring Break
May 25 – Memorial Day
DRESS CODE
- Gymnastics (4-5 Yrs Old): Dancers should wear a shorts-style unitard (no skirts or tutus) and participate barefoot for safety and ease of movement. Hair should be pulled back securely away from the face.
- Combo Classes (5 & Under): Leotard (no detachable skirts), footed tights, tap shoes, and ballet shoes. Dancers may wear any color tap and ballet shoes for class/ Recital Shoe Requirement: White tap shoes and white ballet shoes.
- Combo (5–7 Years): Leotard & tights. Practice shoes any color/Recital requirements: black tap & jazz shoes.
- Ballet (6 & Up): Black leotard, flesh-tone tights & ballet shoes.
- Jazz/Acro: Form-fitting clothes. Flesh-tone So Danca “Jill” or “Jada” jazz shoes.
- Hip Hop: Non-restrictive attire. Sneakers only (No Crocs, boots, Uggs).
- Gymnastics: Form-fitting athletic clothing. Barefoot only.
- Yoga: Bring yoga mat, 2 blocks, sweat towel.
GENERAL POLICIES
Absences & Lateness: No refunds or makeups for missed classes. Students must be picked up promptly to avoid late fees.
Parent Responsibility: Parents must stay informed via studio email, text messages and handouts. Notify the studio of any contact information changes.
Student Care: No before/after-class childcare provided. Escort your child to/from the studio.
Safety: Inform ZFLDC of any health conditions preventing full participation.
Injuries: ZFLDC is not liable for any injury or incident occurring on or off studio premises.
RECITAL DETAILS
Recital Fee: $50 per dancer
Costume Fee: $150 total ($75 deposit + $75 balance)
Fees are auto-drafted if not prepaid.
STUDIO RULES
No dance shoes outside the building
No food/gum in the studio. Water only. Use designated trash bins.
No horseplay in lobby or classroom
Parents must avoid interrupting class
Teachers may dismiss disruptive students
Dismissed students must have a parent/teacher conference before returning
CLASSROOM RULES
Smile and arrive on time
Only small earrings permitted
Water only in class (No juice, soda, etc.)
Disruptive behavior may lead to dismissal from class
COVID-19 LIABILITY
Zeal For Life Dance Company has implemented preventative safety measures to reduce the spread of COVID-19. However, we cannot guarantee that your child will not contract the virus.
By participating in our programs, you acknowledge and agree not to send your child to any on-site programming if they exhibit symptoms such as:
Fever of 99.5°F in the past 72 hours
Persistent cough or difficulty breathing
Chills or repeated shaking
Muscle pain, headache, sore throat
Loss of taste or smell
Diarrhea
If symptoms arise during class, your child will be separated and must be picked up within 15 minutes.
You and your child agree to:
Wear masks when required
Maintain social distancing
Adhere to studio sanitation and hygiene practices
RETURN POLICY FOLLOWING ILLNESS OR EXPOSURE
To return after showing symptoms or testing positive for COVID-19, your child must meet one of the following criteria:
At least 3 days fever-free without medication, symptom improvement, and 10 days since symptoms appeared.
A doctor's note clearing return based on an alternate diagnosis.
Negative test result + symptom improvement.
Notify Zeal For Life immediately if your child has close contact with someone diagnosed or presumed positive for COVID-19.
VIRTUAL LEARNING & REFUNDS
In the event of closure due to COVID-19, classes will continue virtually.
Tuition remains the same for virtual instruction.
No refunds for missed classes, studio closures, or mandatory transitions to virtual learning.
