The undersigned waive and release any and all claims against Perfect Pointe Performing Arts Studio (herein referred to as PPPAS), it's agents, servants, and employees, for any personal injury sustained during participation in any classes, rehearsals, performances, or on the premises of PPPAS.
I certify that my child is in good health and I understand that participation in classes involves physical movement and execution. I understand and agree to adhere to the rules and conduct of PPPAS which are the same for parents and students. If my emergency contact cannot be reached, I give permission to the staff at PPPAS to render aid or to act on my behalf to obtain emergency medical treatment for this student for any illness or injury that may occur while attending classes, rehearsals, performances or on the premises.
I understand that all registration fee are non-refundable. I understand and agree to pay for all classes that I register my child(ten) for at PPPAS, either monthly or in 3 installments and observe the late fee of $20.00 per month should my payment be more then 7 days late.
I understand that I am registering for the full season, September through June, and agree to purchase a recital costume which is non-refundable and non-returnable.
I understand that failure to pay tuition, late fees, costume fees or any miscellaneous charges may result in suspension or termination of my child(ren)s participation in their class(es) until such point that my account is made current. I understand that costumes will not be handed out unless my account is current.
I understand that a student my be removed from recital choreography by the discretion of the teacher if an injury occurs which prevents the student from attending class regularly.
I understand that PPPAS does not issue refunds for any reason, including missed classes due to illness, vacation, religious observation or withdrawal from classes at any point during the year.
I understand PPPAS has permission to utilize any photos of my child for publicity or marketing purposes.
I understand siblings, or children who participate in more than one class, are not guaranteed to be in the same performance at the end of year recital; although the studio will make every attempt to accommodate families.
