You must be at least 18 years old to register for an account.

1. CONSENT TO PARTICIPATE
As the parent or legal guardian of the above-named participant, I hereby grant consent for my child to participate in all activities offered through the Young Dancers Program (YDP) at Bridge for Dance, including but not limited to dance classes (ballet, contemporary, jazz, and hip hop), aerial arts (aerial silks, lyra, and trapeze), rehearsals, and end-of-season performances. I understand that participation in these activities is entirely voluntary.

2. ASSUMPTION OF RISK & LIABILITY WAIVER
I understand and acknowledge that participation in dance, aerial arts, and physical performance activities involves inherent risks, including the risk of physical injury. Aerial arts activities in particular — including aerial silks, lyra, and trapeze — carry elevated risks due to their nature involving height, apparatus use, and physical exertion.
In consideration of my child's participation, I hereby voluntarily release and forever discharge Bridge for Dance, its owners, officers, employees, instructors, and agents (collectively, "Bridge for Dance") from any and all claims, demands, damages, or liability arising from injury, illness, or loss suffered by my child while participating in YDP activities, whether caused by negligence or otherwise, to the fullest extent permitted by law.
I acknowledge that Bridge for Dance makes every effort to maintain a safe environment and well-supervised instruction, and I accept the remaining risks as my own.

3. HEALTH DISCLOSURE & MEDICAL AUTHORIZATION
I confirm that the above-named participant is in good physical health and has no known condition, injury, or impairment that would prevent safe participation in YDP activities. I agree to promptly notify Bridge for Dance of any changes to my child's health status.
In the event of a medical emergency in which I cannot be reached in time, I authorize Bridge for Dance staff to: (a) contact emergency medical services (911); (b) consent to emergency medical treatment on my child's behalf as deemed necessary by medical professionals; and (c) share relevant health information with emergency responders.
I agree to be responsible for all medical expenses incurred by my child in connection with their participation in YDP activities.

4. PHOTO, VIDEO & MEDIA RELEASE
I grant Bridge for Dance permission to photograph and/or record my child during classes, rehearsals, and performances for use in promotional materials, social media, the studio website, press coverage, and documentation of studio activities. No images will be sold to third parties.
Photography and videography by parents, guardians, or visitors is not permitted during YDP classes or rehearsals unless expressly authorized by the instructor. Recordings of performances are permitted for personal use only and may not be shared publicly without written consent from Bridge for Dance.

5. PERFORMANCE PARTICIPATION
I understand that participation in Bridge for Dance's end-of-season performances and showcases is an integral part of the YDP experience. I consent to my child's participation in all rehearsals, dress rehearsals, which may take place at Bridge for Dance or at other venues. I understand that performances may be recorded and shared by Bridge for Dance for promotional purposes (see Section 4).
I acknowledge that costume fees, if any, will be communicated in advance and are the responsibility of the enrolling family.

6. AUTHORIZED PICKUP POLICY
Please pick up your child promptly at the end of class. If someone other than the parent/guardian listed above will be picking up your child, you must notify the front desk in advance. Bridge for Dance will not release a child to any individual not authorized by the parent or guardian on file.
In the event of a late pickup without prior notification, Bridge for Dance reserves the right to charge a late pickup fee and to contact emergency contacts on file.

7. AGREEMENT
By signing below, I confirm that I am the parent or legal guardian of the above-named participant, that I have read and understood all sections of this agreement, and that I agree to all terms set forth herein. I understand this is a legally binding document.

TUITION & FEES:

Tuition is due as scheduled. There are no refunds or credits for missed classes.
Bridge for Dance observes the same major recesses as New York City public schools; no tuition adjustments are made for these closures.
Registration, Recital, and all Processing Fees are non-refundable.

Students may receive a tuition refund only within the first week of classes, minus all fees and the cost of any classes already attended.
No refunds will be issued after the first week of classes begins.
Refunds are not available for students who choose not to participate after the first week of the semester.
If we cancel a class, it will be rescheduled, and all payments will be transferred to the rescheduled class.
Credits (not refunds) may be issued for medical reasons only, valid for one year with required doctor's documentation.

IMPORTANT: Registration is for the FULL YEAR PROGRAM. Those who register are expected to attend both the Fall & Winter/Spring Semesters. This includes performing in both the Winter & Spring Recitals. If a dancer wishes to drop out for the Spring Semester, a $200 drop-out fee will be charged.


ATTIRE & PUNCTUALITY:
Students must arrive on time and dress in appropriate attire for their class. Please refer to the YDP dress code guidelines provided at enrollment.

CLASS OBSERVATION:
Parents and visitors may observe classes during designated observation weeks communicated by the studio. When waiting in reception, please be mindful that classes are in progress.

LOST or STOLEN PROPERTY:
Bridge for Dance is not responsible for lost or stolen personal property.

CLASS CANCELLATIONS:
Bridge for Dance reserves the right to cancel classes due to insufficient enrollment and to make instructor substitutions or faculty changes as needed.

CODE of CONDUCT:
Participants and families are expected to treat all instructors, staff, and fellow dancers with respect. Bridge for Dance reserves the right to dismiss any participant whose behavior is disruptive, unsafe, or otherwise inconsistent with the values of the program.
