Liability Waiver
As a participant or as a parent/guardian of Guelph Youth Dance programs, I recognize and acknowledge that there are certain risks of physical injury and I agree to assuming full risk of any injuries, including damages, death or loss which may be sustained as a result of participation in any activities connected with Guelph Youth Dance. I agree to waive and relinquish all claims against Guelph Youth Dance and its faculty members and volunteers from any and all claims resulting from participation in the program. In case of accidents or sickness I consent to emergency medical care for my child/myself to be provided by ambulance or hospital personnel. I acknowledge the contagious nature of COVID-19 and other contagious diseases and viruses and voluntarily assume the risk that I and/or my children may be exposed to or infected by COVID-19 by attending and participating and that such exposure or infection may result in personal injury, illness, permanent disability and death. I understand that the risk of becoming exposed to or infected by COVID-19 and other contagious diseases and viruses may result from the actions, omissions, or negligence of myself and others, including but not limited to, employees, volunteers and program participants and their families. I have read and understood all of the studios policy information.

Registration fees and Company Deposits are non-refundable.

Class Refunds: If you have started into a session then you will be charged a single class drop in fee ($25) for each 45-60 minute class offered to date, which will be deducted from your refund. Refunds will not be provided after 4 weeks into a session.

Summer Intensive Refunds:
In the event of illness or other unforeseen circumstances, any changes to and/or cancellation of your dancer's registration must be done in writing no later than three weeks prior to the start of the intensive in order to receive a full refund less an administration fee of $50. Refunds requested with less than 3 weeks before the first day of the intensive will be subject to a $150 administration fee. If your dancer becomes ill before camp starts and is unable to attend, the administration fee of $150 will be waived with a doctor's note. Once camp has started, days missed for any reason cannot be refunded or made up
