Waiver & Release of Liability
I understand and agree that participating in any dance class, rehearsal, or performance could involve physical injury. I voluntarily assume all risk and responsibility for injury that could occur to me and/or my child during any said activity by or at DANCE LIVE DREAM CENTER. I agree to release and hold harmless DANCE LIVE DREAM CENTER, including but not limited to its instructors, assistants, and volunteers, from all liability from physical activity that could occur before, during, and after said activities. If I am signing this waiver for my child or children, I certify that I am his/her/parent or legal guardian and have the legal authority to waive these rights.
I grant DANCE LIVE DREAM CENTER permission to use for promotional purposes photographs/videos taken of me and/or my child(ren) during and at DANCE LIVE DREAM CENTER activities.

DLDC promotes kindness, leadhership, discipline, and teamwork. We expect our students and parents to be appropriate and show respect to their teachers and peers at all times. We have a zero-tolerance policy for any form of disrespect from students or parents/guardians. Individuals who display disrespectful, rude, threatening, or conflictive behaviour may be removed from DLDC to maintain a positive and inclusive environment for our community. In order to ensure that all participants and staff are provided a positive, safe and open experience the following “code of conduct” expectations will apply.

Participants and staff are expected to:
1. Follow and adhere to all District, Local, State and Federal Laws, Regulations and/or Local Ordinance with regard to behavior. Parents and guardians of minors are responsible for adherence to District Regulations for those in their care.
2. Participants and Staff are expected to be considerate of others. Treat others with kindness, courtesy, and respect.
3. Refrain from using abusive, obscene, threatening, harassing, insulting or suggestive language or gestures.
4. Refrain from engaging in (or threatening) physical violence, intimidation, assault, or battery, including but not limited to unwanted/unsolicited touching.
5. Protect and respect the facility and grounds of Dance Live Dream Center including equipment, supplies, furnishings and landscaping.
6. Refrain from loitering.
7. Offensive body odors that affect others including the use of scented products or insufficient personal hygiene may result in being asked to leave our studio until offensive odors are eliminated, remedied, or limited.
8. For safety, individuals must be able to care for themselves while partaking in DLDC activities. Staff may determine for the participant’s safety that it is required for the patron to be accompanied by a parent/guardian. The parent/guardian is required to remain with the student during activities and on the premises for the duration of participation.

Thank you for assisting Dance Live Dream Center in providing a positive experience at our dance center. Depending on the nature of violation of the Code of Conduct or DLDC Policy and Procedures, DLDC reserves the right to warn, cite, refer to law enforcement or request the immediate removal of a participant from DLDC property, facility and or program.

DLDC Policy & Procedure Acknowledgment

By enrolling at Dance Live Dream Center (“DLDC”), I acknowledge and agree to the following policies and procedures:

Photography & Videography Consent

I understand that DLDC is a public, performance-based dance studio. By enrolling, I grant permission for DLDC to photograph and video students during classes, rehearsals, performances, and events for promotional, educational, and archival purposes, including use on DLDC’s website and social media platforms.

I acknowledge that photography and videography are a routine part of studio activities and public events. Due to the group-based and public nature of these activities, DLDC does not offer an opt-out option for photography or videography.

DLDC makes every effort to use images respectfully and does not identify students by full name without parent or guardian consent. DLDC is not responsible for images or recordings captured by parents, guests, or third parties at public events.

Social Media Policy

Families, students, staff, and volunteers who identify themselves as affiliated with DLDC are expected to engage on social media with integrity, respect, and professionalism.

When posting images or videos that include DLDC students, identifying information of minors (including full names, ages, or schedules) may not be shared. Content that is obscene, defamatory, threatening, harassing, discriminatory, or offensive is not permitted.

Studio-related matters (including tuition, scheduling, placement, or concerns) may not be discussed via social media and must be communicated directly to DLDC through official channels.

Behavior & Conduct

DLDC maintains a zero-tolerance policy for disrespectful, threatening, or disruptive behavior by students or parents/guardians. DLDC reserves the right to suspend or terminate enrollment without refund if behavior compromises the safety, well-being, or integrity of the studio community.

Financial & Enrollment Acknowledgment

I understand that tuition and fees are non-refundable and that enrollment, payment policies, and participation expectations are outlined in the Dance Live Dream Center Studio Policies.

Policy Updates

I acknowledge that DLDC policies may be updated as needed. The most current policies are available in the Dance Live Dream Center Studio Policies and on the studio website.

By signing below, I confirm that I have read, understand, and agree to abide by DLDC policies and procedures.
