Chesco Dance Center — Agreement for Participation (For Minor Participants)
I, the undersigned parent and/or legal guardian, acknowledge and agree to the following terms regarding my child’s participation in dance classes, rehearsals, performances, and related activities at Chesco Dance Center (“CDC”):
Assumption of Risk
I understand that dance activities may include, but are not limited to: dancing with props, stretching, barre work, across-the-floor exercises, center floor routines, and other physically demanding movements. I recognize that these activities involve inherent risks of physical strain or bodily injury.
I voluntarily assume all such risks associated with my child’s participation.
Waiver of Liability
I understand and accept that CDC and The Gems Dance Company are not responsible for any injuries sustained by my child, nor for any lost, stolen, or damaged personal property.
In consideration of my child being allowed to participate in CDC programs, I voluntarily waive any and all rights to pursue legal action, including claims of negligence, arising from my child’s participation. This waiver applies to CDC, its officers, agents, employees, instructors, subsidiaries, parent organizations, and affiliated entities.
I agree to release, indemnify, and hold harmless CDC from all liability, claims, demands, causes of action, or expenses related to my child’s participation in any CDC activities.
I confirm that I am signing this agreement voluntarily, in full acknowledgment of the risks involved, and in exchange for the opportunity for my child to participate in CDC programs.
Communication
I agree to stay informed by reading all CDC communications and meeting any applicable deadlines.
Program Flexibility
I understand that CDC reserves the right to adjust staff, class times, levels, or formats at any time, for any reason.
Missed Classes & Refunds
No credits, refunds, or exchanges will be issued for missed classes, rehearsals, performances, ticket sales, fees, or costume costs.
Tuition will remain unchanged if classes are transitioned to a virtual format.
No credits are issued for absences due to illness, injury, personal withdrawal, or inclement weather.
In rare circumstances (e.g., pandemic or other emergency), a credit may be issued at CDC’s discretion and applied only to future CDC classes or events.
No cash or check refunds will be issued under any circumstances.
Account Standing
Accounts more than 30 days past due will result in the suspension of the student from all classes and Gems Dance Company events until the account is brought current.
Withdrawal Policy
CDC must be notified at least 31 days in advance to terminate enrollment in classes. Withdrawal requests must be submitted in writing via email by the guardian responsible for the account.
Once withdrawal notification is received, tuition will continue to be charged for two additional months:
⭐ Month 1: The month that includes the required 31-day notice period
⭐ Month 2: The full month immediately following the notice period
This means families are responsible for two final tuition payments after submitting a drop request.
Tuition charges will continue until written/email notification is received. Verbal notice or missed attendance does not stop billing.
CDC accepts cash, checks, and credit cards as forms of payment for classes.
🚨 Important: After March 15 , drops will no longer be permitted, and all remaining tuition for the season will be due in full.
⚠️ Accounts with unpaid tuition balances may be subject to late fees, suspension of class participation, inability to receive costumes or participate in performances/recital, and may be sent to collections. Families are responsible for any additional fees incurred through the collections process.
Tuition charges will continue until proper written notice is received.
Make-up Classes
Students may attend a make-up class of the same or similar level in a different style to compensate for missed classes.
CDC Conduct Expectations
At CDC, we are committed to maintaining a positive, respectful, and uplifting environment for all dancers, families, and staff. We have zero tolerance for gossip, drama, disrespect, or negativity — including inappropriate behavior on social media (such as posts, comments, or reactions) directed at any dancer, parent, or staff member. Any such conduct, by a dancer or parent, may result in immediate dismissal from Gems Dance Company and/or CDC programming. We expect all members of our community to uphold the values of kindness, integrity, and mutual respect at all times.
Acknowledgment
I acknowledge that I have read, understand, and voluntarily agree to the terms of this Agreement for Participation and all CDC studio policies on behalf of my child.
