Agreement for Participation

I understand that dance classes may include, without limitation, dancing with props, stretching, barre work, across the floor combinations, dance routines in the center, and other related activities. I further understand that all of the activities of the dance class involve some degree of risk of strain or bodily injury. Chesco Dance Center is not responsible for personal property.

I agree to be responsible for reading studio correspondence and respecting deadlines, if applicable.

I agree to all tuition guidelines and have read the information provided detailing the payment information and number of payments.

I understand there are no refunds or credits given for missed classes or cancellations for any reason.

As with the transmission of any communicable disease like a cold or the flu, you may be exposed to COVID-19, also known as the “Coronavirus”, at any time or in any place.
Despite our careful attention to sterilization and disinfection, there is still a chance that you could be exposed to an illness in the studio, just as you might be at your gym, grocery store or favorite restaurant. Please know that participation in Chesco Dance Center’s classes and rehearsals is at your own risk.

I hereby acknowledge that I have read the statements above and agree to participate accordingly.

Agreement for Participation

I understand that dance classes, rehearsals, and other activities at Chesco Dance Center may include, without limitation, dancing with props, stretching, barre work, across the floor combinations, dance routines in the center, and other related activities. I further understand that all of the activities of the dance class involve some degree of risk of strain or bodily injury. Chesco Dance Center and the Gems Dance Company is not responsible for any injuries or personal property.
I understand that there are risks of physical injury associated with, arising out of and inherent to the activity of dance. In recognition of this acknowledged risk of injury, I knowingly and voluntarily waive all right and/ or causes of action of any kind, including any and all claims of negligence arising as a result of such activity from which liability could accrue to Chesco Dance Center, it's officers, agents, employees, instructors, subsidiaries, parent corporations, and all affiliated entities Hereinafter collectively referred to as Chesco Dance Center CDC).
I hereby agree to release Chesco Dance Center and hold Chesco Dance Center harmless of all liability, and hereby acknowledge that I knowingly and voluntarily assume full responsibility for all risks of physical injury arising out of active participation in dance on behalf of the participant.
I am aware that this is a release of liability and an acknowledgement of my voluntary and knowing assumption of the risk of injury. I have signed this document voluntarily and of my own free will in exchange for the privilege of participation.
If I am a minor, my parent and / or legal guardian has also signed this document releasing Chesco Dance Center from any and all such liability described above and has acknowledged that I am knowingly and voluntarily assuming all risks of injury inherent to this activity.
I further understand that all of the activities of the dance class involve some degree of risk of strain or bodily injury.
Chesco Dance Center is not responsible for personal property.

I agree to be responsible for reading studio correspondence and respecting deadlines, if applicable.

I understand Chesco Dance Center reserves the right to change staff, class times, and levels at any time for any reason.

I understand there are no credits, refunds, or exchanges made for missed classes, rehearsals, or performances. This include tickets, all fees and costumes for events.

Any account more than 1 month behind in payment will be put on suspension from all classes and Dance Company events until account is paid in full.

In the event that classes will need to go virtual all tuition rates will remain the same.

No credits are given for absences, termination of classes on the student’s part, illness or injury, or
inclement weather. If there is a credit given due to a pandemic or other unforeseen emergencies, it must
be used towards classes or events at CDC.

Cash/check refunds are not available for any reason.

Students are encouraged to make up any missed classes in any style class of the same or near level.

CDC must be notified at least 30 days or 1 month in advance (whichever is less) for termination of
classes via email by the guardian who is in charge of the account. The last tuition fee will be charged for
one month after proper withdrawal notification. Tuition fees will continue to be charged until notification is received in written form or via email.

I hereby acknowledge that I have read the statements above and agree to participate accordingly.

Chesco Dance Center 2025 Recital Participation Waiver

By completing this waiver, I confirm my commitment to participate in Chesco Dance Center's 2025 Dance Recital. I understand and agree to the following:

Regular Attendance: Attendance is necessary for dancers to learn and keep up with recital choreography.

Recital Date: I understand that the recital will take place on Sunday, June 1, 2025.

Dress Rehearsal: I acknowledge that attendance at the dress rehearsal before the performance on June 1, 2025, is required. Missing this rehearsal means I will not be permitted to perform.

Costume Information: I understand that costume information will be provided soon, and fees will not exceed $105 for each costume (including all tights, headpiece, and props). Costumes sometimes require slight alterations or pinning, and are the sole responsibility of parents.

Costume Fees: Costume fees will be due in two payments:
First payment due December 15, 2024. ($52.50 per costume)
Second payment due January 15, 2025. ($52.50 per costume)

Non-Refundable Costume Fees: I acknowledge that costume fees are also non-refundable (no credits).

Tickets- Tickets must be bought in order to attend the recital. Tickets information will be available in the recital packet and will go on sale in May 2025.
