UFAC RELEASE OF LIABILITY AND MEDIA RELEASE WAIVER
I am the student/or undersigned parent or guardian of a student at Urbana Fine Arts Center (hereinafter, UFAC) in Urbana, Illinois where I am/my child is voluntarily participating in dance and other activities offered by UFAC. I fully acknowledge the risk of injury while participating in dance activities or any other activities(whether joining online classes or in-person) offered by UFAC as well any risk associated with traveling to and from these activities, and as consideration for the right to participate in said UFAC activities, I (or myself on behalf of a minor child) knowingly and voluntarily enter into this waiver and release of liability and hereby waive any and all rights, claims or causes of action of any kind whatsoever arising out of my participation or my child's participation in any activity at UFAC, and do hereby release and forever discharge Urbana Fine Arts Group, Inc. (DBA Urbana Fine Arts Center) Luciana Bagby, William Bagby Jr., their affiliates, teachers, guest teachers, managers, members, agents, attorneys, staff, volunteers, heirs, representatives, predecessors, successors and assigns, for any physical or psychological injury, including but not limited to illness, paralysis, death, damages, economical or emotional loss, that I, or my child, may suffer as a direct result of my/my child's participation in the aforementioned UFAC Activity, including traveling to and from an event related to this Activity.
I certify that I/my child/children is/are physically fit, have sufficiently trained for participation in the class(es) we/they are registered for and have not been advised otherwise by a qualified medical person.
I hereby consent to have medical treatment that may be deemed advisable in the event of injury, accident, and/or illness during participation in the program, be given to myself or my child/children/ward.
I agree to indemnify and hold harmless Urbana Fine Arts Group, Inc. (DBA Urbana Fine Arts Center) Luciana Bagby, William Bagby Jr. , their affiliates, teachers, guest teachers, managers, members, agents, attorneys, staff, volunteers, heirs, representatives, predecessors, successors and assigns against any and all claims, suits or actions of any kind whatsoever for liability, damages, compensation or otherwise brought by me or anyone on my behalf, or by me or anyone on behave of the minor child, including attorney fees and any related costs, if litigation arises pursuant to any claims made by me on behalf of myself or my minor child or by anyone else acting on my behalf or on behalf of the minor child.
Further, I give full rights to UFAC and its staff, program holders, producers, sponsors, organizers and/or assigns to use photos and video images of me/my child for promotional purposes pertaining to Urbana Fine Arts Center. Photos and videos may be used in brochures, websites, print advertisements, social media, press releases and other promotional material. Photos may appear with or without names.
I understand and agree to follow the Urbana Fine Arts Center Policies and Procedures listed in the UFAC Student Handbook.
