You must be a parent or guardian to register for an account.

Waiver: We, the staff at PZ Dance Academy, recognize our obligation to make sure our students and their parents are aware of the risks and hazards involved in the sport of dance. By signing this waiver, you release PZ Dance Academy and all its employees from all claims on account of any injury which may be sustained by your child while attending any dance class, event associated with PZ Dance Academy or outside performance, including but not limited to conventions, competitions and recitals. In signing this waiver, you also acknowledge your responsibility in paying monthly tuition, any associated costumes, entry fees for performance, competition, all other communicated costs involved. You also authorize the PZ Dance Academy to charge your credit card on file if payment is not received by the 10th day of the month You also affirm you now have, and will continue to carry proper primary medical, health, hospitalization, accident insurance, which you consider adequate for the protection of both your child and PZ Dance Academy.

1. REGISTRATION:
Registration: A $40 Registration Fee for the first dancer. $30 for the second dancer living in the same household. A family cap of $70. Non-refundable. Must be paid upon registration.
2. TUITION:
The yearly tuition is to be paid monthly (this is divided into nine equal payments regardless of how many classes are in the month, from September through May). Payment is due on the first of the month. If payment is made after the 9th of the month, a $20.00 *late fee will be applied. ALL registered students are required to have a credit card on file for the Auto-Pay transaction. This transaction will happen on the 1st day of the month. If, for any reason, your credit card expires or becomes invalid, it is the parent's responsibility to update this information. If your Auto-Pay fails, you will be notified, and you are required to log into your portal and make the payment. If the payment is late (not paid by the 9th day), you are also responsible for the *late fee (explained above). There are no refunds or transfers of monies paid. If a student is absent, the monthly tuition is not adjusted. The monthly tuition is not adjusted for reasons such as: the month is shorter, the dancer misses class, the dancer is on vacation, etc. These are some examples. If the student withdraws without notification or partway through a current payment period (see policy #3), the monthly tuition is still owed and is not adjusted.
3. WITHDRAWAL:
Withdrawal from the studio must be done in writing. Please see the studio director for a withdrawal form. Otherwise, you are responsible for the current payment period. No refunds or transfers.
4. COSTUME FOR SPRING PERFORMANCE:
These fees will get posted to your dancer's account. Payment for the costumes is expected on November 1, along with the November tuition payment. Ladybugs, Fireflies, and Dragonflies are $110 (one costume), Monarchs are $190 (two costumes). If a dancer is on one of the previously named teams and is in an acro class, that costume is $60. If they only take Acro, the costume fee is $75. If they are in a Hip Hop class, that costume is $95. All recreational Gem teams are $95 for each class/costume. All costumes include a costume, a *recital T-shirt, tights (if needed), and a digital download of the recital. Parents are responsible for the required shoes and undergarments. *Recital T-shirt fee is not included in the Competition Team Costume fees. Those students will get charged separately for the Recital T-shirt.

- Costumes will NOT be ordered for a student unless their costume fee is paid in full.
- In some cases, the dancer and parent may have to buy an additional accessory (ex., Shoes, Ankle socks, headband, hat elastic, etc). You will be notified of this extra purchase, if necessary, once the costume orders have been received.
- Costumes will be given out at the beginning of May on a designated week, and only when costume and tuition accounts (all 9 payments) are paid in full.
- No refunds or transfers. If a student withdraws before the recital performance and all accounts are up to date, they may pick up their dancer's costume after the recital performance is complete.

5. PAYMENT:
You must be enrolled in auto-pay. A processing fee is added. Please note: We will run your tuition on the 1st of every month at no charge. If auto pay fails for any reason, we will notify you via Email. It is then your responsibility to log into your portal and make the monthly tuition payment. You will have 5 days to resolve the issue. In some cases, your account will be put on "inactive" and your dancer cannot return until the account has been settled.

Procedure for Class Arrival and Departure

To ensure the safety and smooth flow of our studio environment, please carefully review and follow the guidelines below regarding drop-off, pick-up, parking, and student preparedness.

DROP-OFF PROCEDURE
Arrival Time: Dancers may arrive no earlier than 5–10 minutes before class begins.

Dress Code: Dancers should arrive ready to dance, with all required dance attire worn under their street clothes. Dressing rooms are available if needed.

Footwear: Street shoes should be easy to remove and put back on—slip-on shoes are ideal.

Accompaniment: If your dancer is under age 9, an adult must walk them into the building.

Water Bottles: Dancers attending class for more than 1.5 hours may bring a labeled, reusable water bottle.

Snacks: No food/snacks unless your dancer has an extended break between classes.

PICK-UP PROCEDURE
Timely Pick-Up: Please ensure the designated adult is in the parking lot at class dismissal time. Staff may be teaching and unable to supervise children who are waiting for their transportation.

For Younger Dancers:

Parents may wait in the lobby during class.

If waiting in your car, please exit your vehicle and meet your child at the door for safe transfer.

For Older Dancers: They may exit the building and walk to their vehicle. Please be sure your dancer knows your family’s plan for pick-up and departure.

PARKING GUIDELINES
Park along the fence or near the studio building, and pull forward to maximize available space.

Please do not park:

In designated No Parking zones

In disabled parking, unless authorized

Along the south side or back of the building—those spaces belong to neighboring businesses (hair salon, tanning salon, church).

Drive very slowly through the lot and stay alert for pedestrian traffic, especially children.

IN-STUDIO EXPECTATIONS
Every dancer should have the following in their clearly labeled dance bag:

Tissues

Hand sanitizer or wipes

Contact information card

CONTACT INFORMATION CARD (Required for Dancers Under Age 10)
Please pin a contact card inside your dancer’s bag that includes:

Name and phone number of the parent/guardian or adult responsible for transportation

This helps instructors contact you quickly in the event that your child:

Becomes ill or shows symptoms of COVID-19 or another illness

Needs assistance or support during class

Annual Spring Performance (Recital)
All students are invited to participate in our Annual Spring Performance. This is a wonderful opportunity for dancers to showcase what they've learned throughout the season.

Participation:
Participation is optional. If you choose not to participate, you must request and submit an opt-out form from the studio director by October 25.

If no opt-out form is submitted by the deadline, your account will be charged for your dancer’s costume, and it will be assumed that they are participating in the performance.

Rehearsals & Attendance:
-Students participating in the recital are required to attend all scheduled rehearsals leading up to the performance.
-Consistent attendance in regular weekly classes is expected.
-All recital participants must adhere to all rules and guidelines related to the performance.

Tickets:
-All audience members, including parents/guardians, must purchase a ticket to attend the performance. Only the participating student is exempt.
-Tickets are sold on a first-come, first-served basis.
Tickets will go on sale 3-4 weeks before the scheduled recital date.

Please note that there are no refunds or exchanges on purchased tickets.

Ticket prices will be announced once the venue is confirmed.
Date & Location:
The performance date and location will be shared with families as soon as the information becomes available.

1. ARRIVAL AND DEPARTURE
No smoking is permitted in or around the building.
All students must wear cover-ups when entering and exiting the studio.
Please be prompt when picking up your dancer. Instruct your child to remain inside the building until picked up.
Once a child leaves the classroom or studio, we are no longer responsible for their supervision.
Parents are welcome to wait in the lobby area during class.
If you bring siblings to the studio, they must be supervised at all times to prevent interruptions.
Children should not run in or around the building, and those who are unusually loud will be asked to sit quietly.

2. COMMON COURTESY
Our staff takes great pride in maintaining a clean, welcoming studio—please help us keep it that way.
Only water is allowed in the lobby.
Please clean up after yourself and your child.
No eating, drinking, or chewing gum is permitted in any of the dance studios.

Do not interrupt instructors while class is in session.
If you have a concern, please email or call the studio director to set up a time to talk.

Student placement is at the discretion of the studio director, and all placement decisions are final.

No cell phone use in the lobby or office area.
All phone calls, video conferences, or video viewing must be taken outside the building to avoid disrupting others.

3. ATTENDANCE
Consistent attendance is essential for student progress.
If your child is too sick to participate, they should stay home and rest.
If injured, they are encouraged to attend class and observe, as this helps them stay informed and engaged in class material.

4. PROMPTNESS
Students should arrive 5–10 minutes before class begins to get ready and avoid disrupting the lesson.
Warm-up is critical for injury prevention, and late arrivals can be unsafe.
Students arriving more than 10 minutes late may be asked to sit and wait for the instructor’s signal to join the class.
If tardiness becomes a recurring issue, you will be contacted by the director to discuss a solution.
If your child knows they will be late, please have them inform their instructor in advance.

5. STUDIO BEHAVIOR
All students and parents are expected to conduct themselves with respect and professionalism at all times, including at events sponsored or attended by the studio.

Inappropriate or disrespectful behavior toward fellow students, parents, or instructors will not be tolerated.
We do not allow negative remarks, comparisons, or gossip about students, parents, or faculty.
A positive, supportive attitude helps foster growth and makes for a more productive environment for everyone.

Student Release
1. Liability: The PZ Dance Academy is not liable for personal injury, loss or damage of personal property. Each student is responsible for informing their instructor(s) of any physical limitation that may prevent full participation in class and may also decline to participate in any activity which they deem harmful.
2. Student Video and Photo Release: I hear by give permission to the video company, the photographer and the PZ Dance Academy Staff to videotape and photograph classes, outside performances, competitions, the dress rehearsals and recital performance in which my child is participating. I understand that these videos and photographs are available for purchase by the attending audience though the video company and photographer. I give the Paula Zawalich Dance Academy permission to use these videos and photos for studio advertisement and the studio website. I give permission for the studio to list my child’s first name only in the advertisements or on the website. The Paula Zawalich Dance Academy is not responsible for the viewing and editing of these video recordings or photographs.
3. Waiver: I, the parent registered participant and or the participant, hereby state that I understand the physical nature of the activity as well as any risk involved, agrees to release, discharge and hold harmless the PZ Dance Academy, its employees and agents from any and all actions, claims, damages, and/or injuries that might occur during a dance class/program and that I, the registered participant, am accustomed to such activity. I will advise the instructor of any physical limitations. All classes and courses must be paid for in full and in advance. I understand there are no refunds or transfers of monies paid.
