Adult Class Registration Agreement
Liability Release
I voluntarily choose to participate in dance classes, workshops, rehearsals,
performances, fitness activities, and other programs offered by Mass Motion Dance. I
understand that dance and physical exercise involve inherent risks, including but not
limited to muscle strains, sprains, falls, and other injuries.
I certify that I am physically able to participate in these activities and assume full
responsibility for my participation. If I have any medical condition that may affect my
ability to participate safely, I understand it is my responsibility to consult with my
physician before participating.
In consideration of being permitted to participate in programs offered by Mass Motion
Dance, I voluntarily release, waive, hold harmless, and agree to indemnify Mass Motion
Dance, its owners, directors, instructors, employees, agents, volunteers, and
representatives from any and all claims, demands, causes of action, liabilities,
damages, costs, or attorney's fees arising out of or relating to any injury, illness, loss, or
damage that may occur as a result of my participation, except where prohibited by law.
I understand that I participate at my own risk and agree to follow all studio rules and
instructor directions to help maintain a safe environment.

Photo & Media Release
Next to this Waiver there is a tab to OPT-OUT of Photos. If you do not authorize Mass
Motion Dance to photograph or record you during classes, rehearsals, performances, or
studio events.
If permission is granted, I understand that photographs and videos may be used for
promotional purposes, including the Mass Motion Dance website, social media pages,
advertising, brochures, and other marketing materials, without compensation.

Adult Class Payment Policy
Adult classes are offered on a pay-as-you-go basis.
Adult Class Rates
 Adult Dance Classes: $12.00 per class
 Adult Tap Classes: $6.00 per class
 10-Class Card: $100.00
Class cards are non-transferable and non-refundable unless otherwise approved by
studio management.
Payment is due at the time of class or prior to participation.
Mass Motion Dance reserves the right to adjust schedules, instructors, or class offerings
as necessary.

Acknowledgment
By signing agreeing to these terms, I acknowledge that:
 I have read and understand this Liability Release and Adult Class Agreement.
 I understand the risks associated with participating in dance and fitness activities.
 I voluntarily agree to the terms outlined above.
 I understand and agree to the studio's payment policies.

Mass Motion Dance Sturbridge

Liability Release and Waiver

I, the parent or legal guardian of the student, hereby give permission for my child to participate in dance lessons, instructional programs, recitals, and other activities offered by Mass Motion Dance.

I understand that dance is a physical activity and, as with any physical activity, participation may involve certain risks, including the possibility of injury.

In consideration of my child’s participation in the programs and activities provided by Mass Motion Dance, I agree to release and hold harmless Mass Motion Dance, its owners, employees, agents, instructors, and officers from any and all claims, demands, actions, causes of action, damages, or expenses, including reasonable attorney fees, arising out of or related to any personal injury my child may sustain as a result of participating in Mass Motion Dance programs or activities, except to the extent caused by circumstances that cannot legally be waived or released.

By signing below, I acknowledge that I have read and understand this Liability Release and Waiver and voluntarily agree to its terms on behalf of myself and my child.

Tuition Payment Agreement & Fees
I understand that by registering for classes at Mass Motion Dance that I commit to paying a monthly tuition on the 1st of each month for a total of ten months, September through June. I must enroll in AUTO-PAY and have a valid card on file throughout the 10 months. I understand a Fee of $25.00 will be applied to my account each month this agreement is not followed.
*Cancellation Policy: Students that decide to stop taking classes throughout the year must provide a 30 day written notice prior to the 1st of the following month. To cancel auto-pay tuition payments, you must provide a 30 day notice in writing to the front desk.
