Teen/Adult- 6 Month Contract
1. Six-Month Enrollment Commitment
By signing this Agreement, the Parent/Guardian agrees to enroll the above-named student in a six (6) month program with Hornsby Martial Arts.
The enrollment is a six-month commitment beginning on the Effective Date and continuing through the End Date. The financial obligation for the six-month term is $300.00, payable according to the payment schedule established by Hornsby Martial Arts.
2. Acceptance of Fee and Payment
The Parent/Guardian understands and agrees that acceptance of enrollment and/or submission of any payment constitutes acceptance of the six-month financial commitment and the terms of this Agreement.
By making the initial payment, providing authorization for automatic payments, or otherwise accepting enrollment, the Parent/Guardian acknowledges that they have agreed to the applicable tuition/fee for the full six-month term.
The Parent/Guardian agrees that the full six-month financial obligation remains due regardless of whether the student attends all scheduled classes, unless otherwise required by applicable law or expressly agreed to in writing by the Hornsby Martial Arts.
3. Cancellation and No-Refund Policy
The Parent/Guardian understands and agrees that all payments made toward the six-month enrollment fee are non-refundable.
If the Parent/Guardian or student chooses to cancel enrollment, discontinue participation, stop attending classes, or otherwise chooses not to complete the six-month term, no portion of any amount previously paid will be refunded, to the fullest extent permitted by applicable law.
Cancellation of enrollment does not automatically cancel or eliminate any remaining financial obligation under this Agreement.
The Parent/Guardian specifically acknowledges that the six-month fee is a commitment for the entire term and is not a month-to-month arrangement.
The "registration" fee is the Tuition for the 6 months, no additional charges will be made to your account for tuition.
