ANNOUNCEMENT
As of July 1st, we will be charging a 3% convenience fee for all online or credit card payments. You will have the option to avoid paying the convenience fee by either dropping off a check at GBA or mailing a check to GBA at PO Box 1561, Greenwich Downtown Station, Greenwich, CT 06836. Your registration will not be final until we receive your payment.

Greenwich Ballet Academy INDEMNIFICATION AND RELEASE
I am the parent/guardian of registered student ("Student"). I hereby agree to allow Student to participate in GBA classes, performances and any related activities organized by Greenwich Ballet Academy, Inc.. GBA is a rigorous ballet training program designed to offer pre-professional ballet training to serious dancers through activities such as: class instruction, performance and other activities at locations within and away from studios. I acknowledge that Student's participation in GBA is voluntary. I hereby attest that I have become familiar with the GBA program as it is described on the GBA website and materials, and agree to all of the terms and conditions set forth on the GBA website and in such materials. I acknowledge that GBA reserves the right to make reasonable changes or modifications to its programs, including but not limited to the schedule, curriculum, faculty and staff, and any other related activities and services.
I attest and verify that Student is free from all illnesses, injuries or defects and that Student is physically and mentally fit and sufficiently trained to participate in GBA. I acknowledge that I have not been advised by a health professional of any risks or conditions that would limit or prevent Student's participation in GBA, and that I am not aware of any medical or other conditions that would limit or prevent Student's participation in GBA. I agree that if the Student sustains any injury or illness prior to the commencement of GBA training, or during the course of GBA training, I will immediately notify GBA in writing so that an assessment may be made as to whether the Student may continue in the program. In the event of any medical condition or injury that can impact ballet training, the Student will provide GBA with a doctor's note and release clearing the Student for dancing and/or outlining any specific modifications. GBA reserves the right to ask for further medical information, should we deem it appropriate and necessary. Withdrawals from GBA are subject to GBA's no refund policy.
I understand that the ballet training offered by GBA may involve touching and adjustment of the Student's bodv by the instructor and that a healthy body conducive to the art of ballet must be maintained to ensure the Student's safety and full participation in GBA.
I understand that Student's participation in GBA's classes and related activities may entail risks to Student's personal safety and/or property, including the risk of serious injury, damage or loss of property, and I knowingly assume all risks associated therewith. I accept full responsibility for any medical expenses incurred during or arising from Student's participation in GBA, including those incurred and not covered by insurance.
I understand that students in GBA programs and their parents/guardians shall be subject to the terms and conditions set forth in GBA's Student Handbook. Copies of the Student Handbook are online at www.greenwichballetacademy.org and hard copies can be made available upon request.
By signing below, I voluntarily do hereby, for myself, Student, my heirs, administrators, executors, assigns, guardians, representatives, and next of kin, forever WAIVE, RELEASE, DISCHARGE, AND COVENANT NOT TO SUE the Greenwich Ballet Academy, and each of its respective affiliates, officers, directors, agents and employees (collectively the "Releasees") for any damage, injury or loss to person or property which may be sustained by Student directly or indirectly as a result of participating in GBA. I further agree to indemnify, defend and hold harmless Releasees from any loss, liability, cost, claim or damages arising from Student's participation in/or association with activities and events organized and sponsored or connected with GBA. I agree that GBA shall not, under any circumstances, be responsible for any indirect or consequential damages, or punitive damages, regardless of cause, and that GBA shall have no liability for any injury or damage suffered or incurred by Student in connection with Student's participation in the GBA program.
This Indemnification and Release shall be controlled, construed and enforced in accordance with the laws of the State of Connecticut without giving effect to conflicts of law principles. I irrevocably submit to the exclusive jurisdiction of the courts of the State of Connecticut located in Fairfield County for the purposes of any suit, action or other proceeding in connection with or relating to this Indemnification and Release or to Student's participation in GBA. In the event that GBA incurs any legal, investigative or other professional fees to enforce the terms of this Indemnification and Release and/or prevail in any dispute with you, GBA shall be entitled to recoup such fees (including, without limitation, reasonable attorneys' fees) from you.
IMPORTANT TUITION INFORMATION
All tuition and fees are payable by check or money order (no credit card payments).
Tuition, including deposits, is non-refundable, for all circumstances.
Placement in class is guaranteed only upon receipt of payment of tuition and fees and class size is capped at a maximum level and do fill up; spots are not guaranteed until payment is received, no exceptions. The faculty reserves the right to change a dancer's level. No student will be permitted to take class until tuition is paid. No refunds will be made after payment.
The registration fee is due once per academic season, prior to the start of classes.
Your invoice for fall and spring season will include a performance fee (which varies by level). The performance fee for each level includes 1 DVD or the performance filmed by a professional production company and other costs associated with the production.
TUITION PAYMENT AGREEMENT:
Greenwich Ballet Academy has a no-refund policy on payments received. I agree to pay the Greenwich Ballet Academy for the dance instruction of the above student per the published registration and tuition rates for the period of study. I understand that I can make payment by check, payable to the Greenwich Ballet Academy, and that there will be a $30 charge for returned checks. I understand that payment of tuition entitles the student to attend ballet classes as scheduled, and that no refunds are given for classes missed because of absenteeism, illness, injury, vacation, or school closings due to acts of God or inclement weather.
Class schedules, the placement of students within classes, levels, and the casting of performances, are subject to change at the sole discretion of the Academy. I understand that tuition fees are due and payable in full by the due date. In some cases of financial need, alternate payment arrangements may be made by contacting the Academy.
If payment is not made by the due date, your place will not be held in class. If payment is not received by the first day of class, your child will not be permitted to take class until such payment is received. There are no tuition refunds for any reason.
POLICY FOR ATTENDING NON-GBA DANCE PROGRAMS:
Students registered in our regular school-year academy may not attend any non-GBA dance classes, programs, summer intensives, private lessons, etc. without the express written permission of the GBA Artistic Director. Students, and their parents, are urged to meet with the Artistic Director to discuss their desire to take classes outside of the Academy. The Greenwich Ballet Academy takes great pride in training its students to the highest standards of instruction available anywhere. We reserve the right to guide our students to programs which will continue to provide the same quality and style of training as our own. Failure to obtain permission to study elsewhere may result in dismissal from the Greenwich Ballet Academy.
