Participation in dance and circus arts involves risk and possible injury. I understand and release Tampa Bay Highland Dancers, its staff, associates, and volunteers, from any claims or medical expenses incurred while participating in classes, rehearsals, workshops, events, or performances.

If medical attention is required for illness or injury, I authorise Tampa Bay Highland Dancers to disclose the above medical conditions to a physician or other medical professional in the event you or your child should require emergency medical care.

I hereby grant Tampa Bay Highland Dancers their successors, assignees, licensees, sponsors the exclusive right to photograph and/or videotape me or my child/ward, and further utilize my and my child or ward’s, name, photo, likeness, voice and appearance in programs, advertising, websites, promotions and for training purposes without reservation or limitation. In granting this license, I understand that Tampa Bay Highland Dancers is not under any obligation to exercise any of its rights, licenses and privileges herein granted.

I understand that the tuition is payable the first week of every month. Each student must be enrolled in auto-pay or a $10 admin fee will be charged monthly. A late fee of 5% will be charged if payment is not received within 14 days.

If you choose to unenroll, you must submit your intentions in writing at least 30 days before your last date. You will be responsible for tuition through that date.

Tampa Bay Highland Dancers follows the Pinellas County Schools Calendar. We will not have classes when the school system is closed. If the school system is closed due to unforeseen circumstances (e.g.: in the case of a hurricane), classes will be cancelled and no refunds will be given.
