WAIVER OF LIABILITY
INJURY, TUITION, AUTO-PAY & PHOTO RELEASE
COLORADO CONSERVATORY OF DANCE
1. Injury Waiver
I acknowledge that participation in dance classes, rehearsals, performances, and related activities at Colorado Conservatory of Dance (CCD) involves an inherent risk of physical injury or illness. I hereby voluntarily assume all such risks. I agree to release, discharge, indemnify, and hold harmless CCD, its employees, faculty, board members, guest artists, agents, and representatives from any and all liability, claims, demands, actions, or causes of action whatsoever arising out of or related to any loss, damage, injury, or illness, including death, that may be sustained by myself or my child while participating in CCD programs or while on CCD premises.
2a. Youth Tuition Policy
The CCD Conservatory season begins in mid-August and includes 38 non-consecutive weeks as outlined in the school calendar.
Enrollment is by semester only (Fall and Spring); monthly enrollment is not available. Each semester requires separate registration.
CCD will be closed during Thanksgiving Break, Winter Break, and Spring Break, as well as on the following federal holidays: Labor Day, Christmas Eve, Christmas Day, New Year’s Eve, New Year’s Day, Memorial Day, Juneteenth, and the Fourth of July.
A one-time, non-refundable registration fee is required per student each academic year, due at the time of initial enrollment regardless of semester.
Tuition is due in full at the time of registration. Families may set up monthly payment plans through the Front Desk; payments are due on the 1st of each month. Auto Pay enrollment is mandatory for all payment plans.
Monthly statements and balances are available via the Parent Portal. Late payments may incur additional fees. Returned checks are subject to a $35 fee. Accounts past due after the 10th of the month may result in immediate suspension from classes until balances are paid in full.
Students must have all accounts current to participate in any CCD production, rehearsal, or performance.
CCD does not provide refunds for paid tuition under any circumstances. Exceptions may be granted in the event of a serious injury, supported by a physician’s note.
Withdrawal from classes must be submitted in writing to the Front Desk by November 15 for the fall semester or April 15 for the spring semester. Tuition remains due for the remainder of the semester if proper notice is not provided.
Participation in CCD’s Annual Showcase is mandatory for all students. A non-refundable Showcase fee of $80 and a costume fee of $50 per costume will be included in spring tuition. These fees are non-negotiable and non-refundable, even if a student later opts not to perform.
2b. Adult Tuition Policy
The Adult Program operates year-round with drop-in classes.
Participants must register and pay for classes through the Parent Portal before attending.
Adult Unlimited Passes auto-renew monthly and must be paid in full on the 1st of each month. Unpaid balances after the 5th may result in removal from the Unlimited Pass and default to drop-in rates.
To cancel the Unlimited Pass, written notice must be submitted to the Front Desk by the 15th of the month prior to renewal.
CCD does not offer tuition refunds. Exceptions may be considered in the event of serious injury with a valid doctor’s note.
3. Registration Fee Policy
An annual, non-refundable registration fee of $40 is due per student. Each additional family member is subject to a discounted registration fee of $30.
4. Auto-Pay Policy
If I am on a payment plan, I understand and agree to enroll in Auto Pay. My credit card will be automatically charged the agreed-upon monthly amount on the 1st of each month. I may cancel Auto Pay through the Front Desk at any time, but any outstanding balance must be paid in full prior to cancellation.
5. Photo and Video Release
I grant permission to CCD to use photographs, videos, or likenesses of myself and/or my child(ren) for promotional purposes in print, online, or other media platforms. I understand that no royalty, fee, or other compensation shall be payable to me for such use. I may opt out of this release at any time by updating preferences in the Parent Portal.
WAIVER OF LIABILITY – ADDENDUM: COVID-19 AND INFECTIOUS DISEASES
COLORADO CONSERVATORY OF DANCE
I acknowledge the contagious nature of COVID-19 and other infectious diseases and voluntarily assume the risk of exposure or infection while attending or participating in CCD classes, events, and performances. Such exposure may result in personal injury, illness, permanent disability, or death. I understand that risk of exposure may result from the actions, omissions, or negligence of myself or others, including but not limited to CCD staff, contractors, volunteers, and participants.
I hereby release and hold harmless CCD, its staff, faculty, contractors, board members, and representatives from all claims related to exposure to or infection by COVID-19 or other contagious diseases.
I agree not to attend or send my child to CCD if anyone in our household is experiencing symptoms of COVID-19 or any other infectious illness, or if anyone has been exposed to a confirmed case. I also agree to comply with all health and safety protocols set by CCD, including exiting the facility immediately if requested by a staff member due to health concerns. I understand that no refunds or credits will be issued for missed classes due to illness or quarantine.
This waiver applies to all CCD programs, classes, performances, and rehearsals at 3001 Industrial Ln. #12, Broomfield, CO 80020, and all off-site venues. This addendum does not replace the general waiver of liability, which remains in effect.
