I hereby grant Atelier88 Performing Arts, LLC, its owners, operators, affiliates, and licensees permission to use and reproduce images or videos of my child in any format for promotional purposes, including but not limited to marketing, advertising, print materials, social media, websites, and internet advertising. I acknowledge that these images and videos are the property of Atelier88 Performing Arts, LLC. I understand that no financial compensation will be provided for their use. Additionally, I agree to fully indemnify, hold harmless, and release all claims against Atelier88 Performing Arts, LLC. regarding the use of these images and videos.

I acknowledge that I am required to keep a valid credit card on file and make payments by the due dates. I understand that a $15 late fee will be applied if payment is not made within 5 days of the due date. Additionally, I am aware that a $25 administrative fee will be charged for each month a valid card is not on file. If my credit card is declined multiple times, a $15 processing fee will be incurred for each decline. I also understand that my child will not be allowed to attend class if there is an outstanding balance on my account.
