I, representing both myself and the children listed above, hereby acknowledge and consent to the following terms and conditions pertaining to their involvement in activities at Upper Valley Cheer:
1. **Assumption of Risk:** I recognize and understand that cheer activities inherently involve risks and dangers, encompassing physical injury and emotional distress. I willingly and voluntarily accept these inherent risks on behalf of my child.
2. **Release of Claims:** On my behalf and for my children, I hereby release, waive, discharge, and covenant not to sue Upper Valley Cheer, including its owners, employees, instructors, and agents, from any and all liability arising from personal injury, death, or property damage resulting from my children's participation in cheer activities.
3. **Indemnification:** I agree to indemnify and hold Upper Valley Cheer harmless from any claims, demands, actions, or liabilities, including legal fees, arising from my children's participation in cheer activities.
4. **Emergency Medical Treatment:** In the event of an emergency, I authorize Upper Valley Cheer volunteer staff to obtain any necessary medical treatment for my children. I understand and accept responsibility for any incurred medical expenses.
5. **Photographic Release:** I grant Upper Valley Cheer the right to use photographs and videos of my children taken during activities for promotional and marketing purposes.
I have thoroughly read and comprehended this Liability Waiver and Release Form, and I willingly sign it to permit my children's participation in all activities at Upper Valley Cheer.
