1. Acknowledgment of Risk
I understand and acknowledge that dance is a physical activity and sport that involves inherent risks, including but not limited to muscle strains, sprains, falls, collisions, fractures, and other serious injuries. I understand that these risks may arise from participation in classes, rehearsals, performances, conditioning, choreography, use of equipment, or interaction with other participants.

I voluntarily choose to participate (or allow my child to participate) in activities at Radiate Dance Company with full knowledge and acceptance of these risks.

2. Release of Liability
In consideration of being permitted to participate in activities at Radiate Dance Company, I, on behalf of myself and/or my child, hereby release, waive, discharge, and hold harmless Radiate Dance Company, its owners, directors, instructors, staff members, assistants, contractors, and affiliates from any and all liability, claims, demands, or causes of action arising out of or related to any injury, illness, damage, or loss sustained as a result of participation in any Radiate Dance Company activity, whether caused by negligence or otherwise, to the fullest extent permitted by law.

3. Medical Authorization
I certify that I (or my child) am physically able to participate in dance activities. I understand that it is my responsibility to inform Radiate Dance Company of any medical conditions, injuries, or limitations.

In the event of an emergency, I authorize Radiate Dance Company staff to seek medical treatment for myself or my child if I cannot be reached. I understand that I am financially responsible for any medical care provided.

4. Personal Property
Radiate Dance Company is not responsible for any lost, stolen, or damaged personal items brought onto studio premises or to studio-related events. Participants are encouraged to leave valuables at home and clearly label personal belongings.

5. Photo, Video, and Media Release
I grant Radiate Dance Company permission to photograph and/or record myself and/or my child during classes, rehearsals, performances, and studio activities. I understand that these images and recordings may be used for purposes including, but not limited to:
Studio marketing and promotional materials
Website and social media
Printed materials and advertisements
Studio displays and publications

I understand that no compensation will be provided for use of these images or recordings.

6. Assumption of Responsibility
I agree to follow all studio policies, rules, and safety instructions provided by Radiate Dance Company and its staff. I understand that failure to follow safety instructions may increase the risk of injury.

7. Acknowledgment and Agreement
I have carefully read this Waiver and Release of Liability Agreement and fully understand its contents. I understand that I am giving up certain legal rights by signing this document. I sign this Agreement voluntarily and without coercion.

Radiate Dance Company is committed to providing consistent, high-quality instruction and maintaining a safe environment for all dancers.

In the event of severe weather, natural disasters, public health emergencies, epidemics, pandemics, government orders, utility outages, facility damage, or other circumstances beyond the control of Radiate Dance Company (including but not limited to acts of God, emergency closures, or unsafe travel conditions), the studio may temporarily close or modify operations.

When possible and appropriate, Radiate Dance Company will implement alternative instructional methods, including virtual classes, video instruction, or other remote learning options, to ensure continuity of training.

Tuition remains due and non-refundable during temporary closures, as enrollment reserves the dancer’s place in class and supports ongoing operational expenses, instructor compensation, and continued instructional delivery.

Radiate Dance Company will make reasonable efforts to communicate closures and alternative arrangements promptly through email, the Parent Portal, and Band App.

By enrolling, families acknowledge and agree to this policy.

Tuition & Payment Policy
• Tuition is charged on a monthly basis and reserves your dancer’s place in class.
• Tuition is not based on attendance and is not prorated for missed classes.
• Tuition remains the same regardless of the number of classes in a given month.
• All tuition and fees must be paid through the Parent Portal using the secure payment system.

Tuition covers:
• Instruction
• Studio operations
• Instructor compensation
• Facility expenses
• Ongoing program development

Auto-Pay Policy
Radiate Dance Company requires a valid payment method on file for all enrolled families. Cash or check payments made on or before the due date will not result in an automatic charge to the card on file.
By enrolling, you authorize Radiate Dance Company to automatically charge your card on file for:
• Monthly tuition
• Registration fees
• Costume fees
• Competition fees (if applicable)
• Other authorized studio-related charges

Additional Auto-Pay Terms:
• Payments are processed automatically on the designated billing date.
• It is the account holder’s responsibility to maintain a valid payment method.
• Declined payments must be resolved promptly to avoid late fees or enrollment interruption.
• Radiate Dance Company uses secure payment processing systems to protect your financial information.

Late Payment Policy
• Accounts with declined or unpaid balances may be subject to late fees.
• Accounts must remain in good standing for dancers to participate in classes, rehearsals, competitions, and recital.
• Radiate Dance Company reserves the right to pause participation until balances are brought current.

If you are experiencing difficulty making a payment on time, please contact the office prior to the due date. We are happy to work with families to set up a payment plan when needed.

Radiate Dance Company accepts cash, check, ACH bank payments and credit/debit card payments for your convenience.

Radiate Dance Company requires written notice to withdraw a dancer from classes.

• Withdrawal requests must be submitted in writing via email to admin@radiatedance.com.
• Withdrawal becomes effective the following billing cycle after notice is received.
• Tuition is not refunded for prior months or missed classes.
• Tuition continues to be charged until written withdrawal is received and processed.

Enrollment reserves your dancer’s place in class. Without written withdrawal, enrollment and tuition remain active.
