In the event that I decide to withdraw my child from classes at ZD Dance Academy, I understand that I must submit a written withdrawal request to the studio at least 30 days prior to the effective withdrawal date. This written request must also be accompanied by the dropping of said class(es) through the online portal.

I understand that my child will remain enrolled and eligible to attend classes during the 30-day withdrawal notice period, and all tuition policies shall continue to apply until the effective withdrawal date. I acknowledge that I am responsible for all charges billed to my account within the thirty days immediately following my written withdrawal request and online class drop.

This includes, but is not limited to, registration fees, tuition fees, late fees, NSF fees, costume fees and any other outstanding charges related to my child's enrollment at ZDDA. I agree to settle any outstanding balance in a timely manner and ensure that the provided debit or credit card on file remains valid during this period.

If the account balance is not fully cleared within the specified 30-day withdrawal period, I acknowledge that ZD Dance Academy reserves the right to pursue legal action to recover the outstanding fees. In such a case, I understand and agree that I will be solely responsible for all attorneys' fees, court filing fees, and any other legal costs incurred by ZD Dance Academy in the process of recovering the outstanding balance.

I further understand that this legal action may have potential consequences on my credit and may affect my ability to enroll my child in future classes or events at ZDDA.

I have read and accept the 30-day withdrawal policy and agree to be responsible for any and all charges billed to my account within this period after submitting my written withdrawal request.

I acknowledge and agree to be responsible for the purchase of costumes to be worn by my child in recitals, showcases, and other studio events at ZD Dance Academy. I understand that I will be financially responsible for all costumes ordered for my child, even in the event that they do not participate in the specific event that warranted the costume purchase.

I further understand that the costume fees must be paid by the given deadline specified by ZD Dance Academy. Failure to make timely payment may result in the addition of late fees, rush production fees, and expedited shipping costs to my account.

I am aware that costumes will be withheld from my child until all costume fees and associated costume charges are paid in full. I agree to settle any outstanding costume-related expenses promptly and ensure that the provided debit or credit card on file remains valid for any necessary payments.

By accepting this waiver, I confirm that I have read and understood the costume responsibility clause, and I accept the financial obligation for all costumes ordered for my child's participation in studio events.

The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. Zion’s Daughters of Dance, Inc DBA ZD Dance Academy has put in place preventative measures to reduce the spread of COVID-19; however, ZD Dance Academy cannot guarantee that you will not become infected with COVID-19. Further, participation could increase your risk of contracting COVID-19.


By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I or my minor child may be exposed to or infected by COVID-19 by participation; and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at ZD Dance Academy may result from the actions, omissions, or negligence of myself and others, including, but not limited to, ZD Dance Academy’s employees, volunteers, and program participants and their families.


I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to myself and my minor child (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I may experience or incur in connection with my participation at ZD Dance Academy. On my behalf, I hereby release, covenant not to sue, discharge, and hold harmless ZD Dance Academy, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any claims based on the actions, omissions, or negligence of ZD Dance Academy, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation at ZD Dance Academy.


I represent that both my minor child and I have adequate insurance to cover any injury or illness we may suffer or cause while participating in this activity, or else I agree to bear the costs of such injury or illness myself. I further represent that my minor child and I have no medical or physical condition which could interfere with my safety in this activity, or else I am willing to assume – and bear the costs of – all risks that may be created, directly or indirectly, by any such condition.


By signing this document, I agree that if my minor child or I am exposed or infected by COVID-19 during my participation in this activity, then I may be found by a court of law to have waived my right to maintain a lawsuit against the parties being released on the basis of any claim for negligence.


I have had sufficient time to read this entire document and, should I choose to do so, consult with legal counsel prior to signing. Also, I understand that this activity might not be made available to me or that the cost to engage in this activity would be significantly greater if I were to choose not to sign this release, and agree that the opportunity to participate at the stated cost in return for the execution of this release is a reasonable bargain. I have read and understood this document and I agree to be bound by its terms.


I agree that my minor child and I will practice safe social distancing and clean hygiene during my participation at ZD Dance Academy.

I hereby authorize the staff of ZD Dance Academy and its associates to act on my behalf according to their best judgement in any emergency situation requiring medical attention and I hereby waive and release ZD Dance Academy and its associates from any and all liability for any injuries or illness incurred while attending classes at the studio or on the studio premises. In addition, permission is granted for my child to be photographed/video recorded on ZD Dance Academy Premises. Permission is also granted for any of the photographs or videos taken to be used in marketing materials to promote ZD Dance Academy.

ZDDA Season 12 Payment Policies & Procedures

I hereby acknowledge and agree to the tuition policies set forth by ZD Dance Academy for the 2026-2027 dance season. By signing this waiver, I understand and accept the following terms and conditions:

1. Payment Schedule: I understand that a registration fee and the initial tuition installment payment is due at the time of registration. Yearly tuition rates are based on the number of weekly class hours for which my child is enrolled.

2. Payment Due Date: I will ensure that tuition is paid in full on or before the 20th day of each month. A five-day grace period will be granted until the 25th of the month, after which payments will be auto drafted.

3. Acceptable Payment Methods: I understand that ZD Dance Academy accepts tuition payments via Zelle (preferred), PayPal, Cash App, Stripe, and the DanceStudio-Pro (DSP) Online Payment Portal. I understand that payments made through the DSP Payment Portal (online or in-person) are subject to a $9.95 convenience fee per transaction. Parents choosing to make manual payments are responsible for ensuring that payment is received on or before the monthly due date. Any tuition balance that remains unpaid after the five-day grace period will be automatically charged to the debit or credit card on file in accordance with the Academy's Automatic Debit Policy.

4. Automatic Debit System: I agree to provide a valid debit or credit card for automatic payments, and I authorize ZD Dance Academy to charge the provided card for all tuition installments and associated fees.

5. Late Fees: If payment has not been successfully processed by the 1st day of the active month, I understand that a $20 Late Fee will be charged to my account. Additional late fees ($20) will be added on the 7th day and again on the 14th day of the active month for any remaining unpaid balance.

6. Non-Sufficient Funds (NSF) Fee: I acknowledge that a $35 NSF Fee will be added to my account if a payment is declined.

7. Class Attendance: I understand that my child will not be allowed to attend class if there is a past-due tuition balance as of the 20th day of the active month. To ensure my child's participation, I will make sure the account is brought current, including all past due tuition charges and associated fees.

8. Prorated Tuition: I am aware that tuition will be prorated for students enrolling during an active month.

9. Refund & Transfer of Payments: I understand that all payments made to ZD Dance Academy are both NON-REFUNDABLE and NON-TRANSFERABLE including, but not limited to, registration fees, acceptance deposits, choreography fees, tuition payments, costume fees, production fees, activity fees, merchandise costs and all other studio related charges.

10. Fundraiser Clause: I am aware that (1) All funds generated through fundraiser efforts will be distributed directly to ZDDA and applied to your student's account to cover studio-related costs included, but not limited to, tuition, costume fees, fundraiser fees, late fees, and
other associated ZDDA expenses. In the event of outstanding balances, the studio owner may elect to apply your fundraiser proceeds to your past due balance before distributing it to current expenses; 2) All funds are non-transferable and are not redeemable for cash and; (3) All funds are immediately forfeited in the event a student withdraws or is dismissed from ZD Dance Academy.

11. Legal Action: If my account is unpaid and inactive for a period longer than thirty days I acknowledge that ZD Dance Academy reserves the right to pursue legal action to recover the outstanding fees. In such a case, I understand and agree that I will be solely responsible for all attorneys' fees, court filing fees, and any other legal costs incurred by ZD Dance Academy in the process of recovering the outstanding balance. I further understand that this legal action may have potential consequences on my credit and may affect my ability to enroll my child in future classes or events at ZDDA.

I have read and understood the above payment policies, and I agree to comply with them throughout my child's enrollment at ZD Dance Academy. I further authorize ZD Dance Academy to charge my provided debit or credit card for all tuition-related charges.

SPOT TV VIDEO SURVEILLANCE, PHOTOGRAPHY & MEDIA RELEASE WAIVER

I acknowledge that I have voluntarily enrolled my child(ren) at ZD Dance Academy, located in Lanham, Maryland (hereinafter referred to as "ZD Dance"). I understand and accept that ZD Dance utilizes a webcam-based video surveillance system for instructional, training, safety, security, and management purposes. I further understand that authorized live and recorded classroom access may be provided through the Spot TV platform (hereinafter referred to as "Spot TV") and is subject to Spot TV's Terms and Conditions.

By accepting this waiver, I grant ZD Dance and Spot TV permission to photograph, videotape, livestream, and record the image, likeness, and voice of myself and/or my minor child(ren), as applicable, while participating in classes, rehearsals, performances, and other activities conducted by ZD Dance.

I understand and agree that all photographs, video recordings, livestreams, audio recordings, and related media (collectively, the "Media") are the sole property of ZD Dance and/or its authorized service providers. I acknowledge that I have no ownership rights or intellectual property interest in any Media created through this program.

I further agree that I will not, nor will I permit my child(ren) or any other person using my account to, capture, record, photograph, screenshot, download, copy, reproduce, transmit, distribute, publish, edit, modify, display, sell, share, or create derivative works from any Media or Spot TV content without the prior written consent of ZD Dance. I understand that any unauthorized use, reproduction, or distribution of copyrighted Media may violate federal copyright law and may result in the immediate termination of my Spot TV access, removal from the program, and/or legal action.

I acknowledge that I am solely responsible for maintaining the confidentiality and security of my Spot TV login credentials and for all activity occurring under my account. I accept full responsibility and liability for any unauthorized recording, copying, or distribution of Media associated with my account, whether performed by me, my child(ren), or any individual granted access through my credentials.

For purposes of this waiver, the term "Media" includes, but is not limited to, photographs, digital images, video recordings, livestreams, motion picture footage, and audio recordings.

I understand that ZD Dance and/or Spot TV reserve the right to enforce their intellectual property rights and pursue any legal or equitable remedies available in response to violations of this agreement.
