Tous les élèves doivent remplir ce contrat afin de participer à tout cours offert par l’Académie de Danse Elite Inc. Si l’élève est âgé de moins de 18 ans, un parent ou un tuteur doit également signer ce contrat. L’acceptation électronique de cette décharge est considérée comme juridiquement valable.
Décharge de responsabilité
Je reconnais et comprends les risques de blessures physiques inhérents à la danse et à l’entraînement en danse, et j’assume pleinement ces risques. Par la présente, je dégage Shannon-Lee Figsby, le studio, ses employés et ses contractuels de toute responsabilité pour les blessures subies ou maladies contractées lors de la participation à tout cours de danse, répétition, atelier ou spectacle. J’accepte d’indemniser, de défendre et de dégager de toute responsabilité Shannon-Lee Figsby, le studio de danse, ses employés et ses contractuels pour toute responsabilité, tout coût et tout jugement résultant d’actes ou d’omissions commis par moi ou mon enfant, qui causeraient des blessures ou des dommages à toute personne ou à tout bien.
Soins médicaux
J’autorise le studio à prendre toutes les mesures raisonnables pour répondre à une urgence médicale ou autre, y compris, mais sans s’y limiter, l’administration de premiers soins et l’obtention d’une assistance médicale professionnelle. J’autorise le studio à prendre les dispositions nécessaires pour transporter moi-même ou mon enfant vers un établissement médical si nécessaire. Tous les frais associés à ce transport et à ces soins médicaux seront à ma charge exclusive.
De plus, je m’engage à informer mon enfant d’aviser immédiatement les instructeurs en cas de douleur et/ou d’inconfort important pendant toute activité. Si mon enfant se plaint de douleur et/ou d’inconfort important après une activité, je m’engage à en informer le studio dès que possible.
Biens personnels
Je comprends et accepte qu’il est de ma responsabilité exclusive de protéger mes biens personnels pendant ma participation à des cours, répétitions, ateliers ou spectacles. Je dégage par la présente Shannon-Lee Figsby, le studio, ses employés et ses contractuels de toute responsabilité en cas de perte ou de dommage à mes biens personnels durant ces activités.
Mandats gouvernementaux
En cas de fermeture imposée par le gouvernement, toutes les dates inscrites au calendrier du studio pourront être modifiées et/ou prolongées afin de respecter le contrat de 30 semaines, ou, en dernier recours, annulées. En cas de fermeture gouvernementale, tous les cours passeront immédiatement sur Zoom sans modification des frais. Aucun remboursement ne sera effectué pour les frais liés aux compétitions, spectacles, costumes, cours ou leçons de danse si ceux-ci ne peuvent avoir lieu en partie ou en totalité en raison d’un mandat gouvernemental.
Circonstances imprévues
En cas de circonstances imprévues hors de notre contrôle (ex. : pandémie, inondation, incendie, annulation par une autorité supérieure, etc.), toutes les dates inscrites au calendrier du studio pourront être modifiées et/ou prolongées pour respecter le contrat de 30 semaines, ou annulées. Si possible, les cours seront immédiatement transférés sur Zoom sans modification des frais. Aucun remboursement ne sera effectué pour les coûts liés aux compétitions, spectacles, costumes, cours ou leçons de danse si ces derniers sont annulés en tout ou en partie en raison d’une circonstance imprévue.
Forfait spectacle
Je, soussigné(e), accepte d’acheter un forfait spectacle de fin d’année comprenant un costume de spectacle, un enregistrement vidéo et une photo de groupe. Je comprends que ce forfait est non remboursable. Aucun remboursement ne sera accordé en raison de la nature intime des costumes techniques (non retournables) et du fait que les services de vidéographie et photographie doivent être réservés à l’avance.
Les élèves inscrits à plusieurs cours ne recevront qu’un seul forfait incluant les vidéos. Tous les clients recevront automatiquement une copie des deux spectacles et leur photo de groupe professionnelle.
Règlement sur l’uniforme
Je, soussigné(e), m’engage à respecter les règlements d’uniforme de l’Académie Elite et à acheter tout article nécessaire supplémentaire pour que mon enfant puisse continuer à participer à son cours. Les achats peuvent être effectués ailleurs qu’à Elite. Les élèves portant un uniforme incorrect pourraient se voir refuser l’accès au cours si leur tenue représente un risque de blessure pour eux-mêmes ou les autres.
Paiement automatique
Tous les paiements doivent être effectués par prélèvement automatique (AutoPay). Des frais de retard de 5 % seront automatiquement ajoutés à tout solde en souffrance de 30 jours ou plus. Nous nous réservons le droit de refuser les services à toute famille dont le compte est en souffrance de plus de 30 jours. Tout compte en souffrance de 60 jours sera immédiatement confié à une agence de recouvrement.
Afin de maintenir la qualité de nos cours, aucun remboursement ne sera accordé après le 1er novembre 2025. Les clients demeurent responsables des paiements de frais de scolarité convenus, que l’élève assiste ou non au cours. En cas d’expulsion de l’élève du studio, aucun remboursement pour le mois en cours ne sera effectué. Tous les paiements et frais de costumes sont non remboursables, incluant mais ne se limitant pas aux vacances, maladies ou blessures.
Frais de scolarité
Les frais de scolarité sont payés en dix versements pendant la saison. Ils sont prélevés automatiquement le 1er de chaque mois du 1er octobre 2025 au 1er juin 2026, suivant un premier paiement dû lors de l’inscription. Les frais varient selon la durée ou le niveau du cours et sont indiqués dans le portail d’inscription.
Les remboursements pour raisons médicales uniquement après novembre 2025 pourront être envisagés selon deux options (au choix des parents) :
Crédit, moins le coût des cours suivis et les frais administratifs de 10 %
Remboursement, moins les frais d’inscription de 35 $ pour l’ouverture du portail parent, moins le coût des cours suivis et une pénalité de 10 %
Si nous ne pouvons pas prélever votre compte le 1er du mois, une seconde tentative sera faite le 5. Une troisième et dernière tentative aura lieu le 15. En cas d’échec, vous devrez vous présenter au bureau pour effectuer le paiement en espèces.
Autorisation de diffusion médiatique
L’Académie de Danse Elite Inc. se réserve le droit d’utiliser les photographies et vidéos prises pendant les cours, ateliers, spectacles ou événements affiliés à des fins d’enseignement, de publicité et de promotion de ses programmes. Les élèves ou les parents d’élèves mineurs qui ne souhaitent pas se conformer à cette politique doivent en informer l’Académie avant le début de la session en sélectionnant l’option « Ne pas publier » lors de l’inscription.
En acceptant ce contrat, j’accepte les conditions et dispositions qu’il contient et reconnais comprendre les modalités des services offerts par l’Académie de Danse Elite Inc.

All students must complete this contract in order to participate in any classes at Academie de Danse Elite Inc. (“the studioâ€). If the student is under the age of 18, a parent or guardian must also sign this contract. Electronically accepting this waiver is considered legally acceptable.
Waiver of Liability
I recognize and understand the risks of physical injury inherent to dance and dance training and I fully assume those risks. I hereby release Shannon-Lee Figsby, the studio, it’s employees and contractors from all liability for injuries sustained or illnesses contracted while attending or participating in any dance classes, rehearsals, workshops, or performances. I agree to indemnify, defend, and hold harmless Shannon-Lee Figsby, the Dance Studio, employees and contractors for liabilities, costs and judgments arising from acts of omissions committed by me or my child which result in injury or damage to any person or property.
Medical attention
I authorize the studio to take all reasonable steps to respond to a medical or other emergency, including but not limited to the providing of immediate first aid and obtaining professional medical assistance. I authorize the studio to make necessary arrangements to transport myself or my child to a medical treatment facility if ever necessary. All costs and expenses associated with such transportation and medical treatment will be at my sole cost and expense.
Additionally, I agree to instruct my child to notify instructors immediately of any pain and/ or major discomfort felt during any activity. If my child complains of any pain and/or major discomfort following an activity, I agree to inform the studio as soon as possible.
Personal Property
I understand and agree that it is my sole responsibility to safeguard my personal property while attending or participating in any classes, rehearsals, workshops, or performances. I hereby release Shannon-Lee Figsby, the studio, it’s employees and contractors from all liability for loss or damage to my personal property while attending or participating in classes, rehearsals, workshops, competitions or performances.
Government Mandates
In the event of a government-mandated shutdown, all dates listed on the studio calendar may be changed and/or extended in order to fulfill the 30-week contract, or ultimately cancelled. In the event of a government-issued shutdown, all classes will immediately shift to Zoom without fee modification. No refunds will be issued for any costs including and related to dance competitions, dance recitals, dance costumes, dance classes and/or dance lessons should any of the above mentioned fail to produce due in-part or in totality of a government-mandate.
Unforeseen Circumstances
In the event of unforeseen circumstances beyond our control (ex: pandemic, flood, fire, cancellation by a higher authority, etc.) and until the resolution of the situation, all dates listed on the studio calendar may be changed and/or extended in order to fulfill the 30-week contract, or ultimately cancelled. If possible depending on nature of circumstance, all classes will immediately shift to Zoom without fee modification. No refunds will be issued for any costs including and related to dance competitions, dance recitals, dance costumes, dance classes and/or dance lessons should any of the above mentioned fail to produce due in-part or in totality of an unforeseen circumstance.
Recital Package
I, the undersigned, agree to purchase an end-of-year recital package including a recital costume, video recording and group photograph. I understand that my recital package is non-refundable. Absolutely no refunds will be issued whatsoever due to the fact that the intimate nature of most technical costumes render them unacceptable for returns, and videography and photography contractors must be booked in advance.
*Students and families with children in more than one class will only receive the full package charge including video recordings ONCE. All clients will receive a copy of both shows and their professional group photograph automatically.
Uniform Policy
I, the undersigned, agree to ensure Elite’s uniform regulations are respected, and purchase any additional necessary items for my dancer to continue participating in their class. Purchases do not need to be made through Elite but can be. Students attending class in an incorrect uniform may be asked to sit out if their attire poses a risk of injury to themselves or others.
AutoPay
All payments must be made by AutoPay. A 5% late fee will be automatically added to the balance of any accounts that are 30 or more days past due. We reserve the right to deny services to any family with an account balance over 30 days past due. Any account that reaches 60 days past due will be immediately forwarded to a collection agency. In order to maintain the quality and integrity of our dance classes, no refunds will be given after November 1, 2023. Clients remain responsible for their agreed upon tuition payments regardless of whether the student attends class. In the event a student is expelled from the studio, no refund for the current month will be made. All payments and costume fees are non-refundable for any reason, including, but not limited to vacation, illness and injury.
Tuition and Fees
Tuition Fees are paid in ten instalments throughout the season. Tuition Fees are paid via AutoPay on the first of every month from October 1st, 2023 to June 1st 2024, following the first instalment that is due at registration. Tuition fees are listed next to classes per month in registration portals and differ depending on the length or ability level of course. Refunds for medical reasons ONLY after November 2023 may be considered, and will be processed via one of two options (onus of choice on parents.)
1. Credit, minus the cost of the classes taken and the administrative fee of 10%
2. Refund minus the $35 Registration fee to open a Parent Portal, minus the cost of the classes taken and the penality fee of 10%.
If for any reason we are not able to charge your account on the 1st of the month, a second AutoPay run will be taken on the 5th. A third and final run will take place on the 15th, after which you must visit the office and make your payment in cash should it be refused for a third time.
Refused pre-authorized payment or cancelled payment
Service fees of $25 will be applied for any pre-authorized payment that has been refused (expired or invalid credit card, etc.)
Service fees of $50 will be applied for any pre-authorized payment that has been refused (expired or invalid credit card, etc.) for a second offence. Fees of $75 will be charged for all contested and/or stopped payments by the customer where the customer has not attempted to contact Elite either by email at a valid email address or by telephone (and have left a message.)
Media Release
Academie de Danse Elite Inc. reserves the right to use photographs and videos taken during classes, workshops, performances, or other affiliated events for the purposes of instruction, advertising and promoting Academie de Danse Elite Inc. and its programs. Students, or parents of students who are minors, who do not wish to comply with this policy must notify Academie de Danse Elite Inc. before the session starts by choosing the “Do Not Post Publicly†option during registration. Notify the studio immediately if your situation changes.
Behavioural Considerations, Commitment to Fostering A Positive Dance Environment, Commitment to be a Partner in Dance:
I understand that participation in any program at Elite requires a commitment from both the student and parent. Students missing excessive amounts of class time may be asked to take a private lesson to catch up. I understand that participation in a group activity requires a baseline of acceptable behaviour and Elite is limited in it’s resources to work with students who struggle to respect their peers or instructors. I agree to keep Elite a positive, welcoming atmosphere to all who enter it’s doors, and I agree to limit any gossip, negative speech or otherwise unwelcome factors completely. I understand that if at any time, the behaviour of my child or myself is considered to be unscrupulous and undesirable, myself or my child can be dismissed without warning and will not receive a refund for any fees paid up until said dismissal. I understand that dance lessons should be fun, a dance studio should be warm, communicating should be pleasant and I agree to do my part in keeping it that way by being a true Partner in Dance to both my child and the studio.
By accepting the present contract, I agree to the terms and provisions contained in this contract and acknowledge I understand the terms and conditions of services engaged through Academie de danse Elite Inc.
