Photography, Video, and Media Release

Straight Up Dance Academy ("SUDA") may photograph, video record, livestream, or otherwise capture images, audio, or video of students during classes, rehearsals, performances, competitions, camps, workshops, special events, and other studio-related activities.

Unless I notify Straight Up Dance Academy in writing as provided below, I grant SUDA permission to use my child's (or my) name, photograph, image, likeness, voice, video recordings, and performance footage for lawful educational, promotional, advertising, marketing, archival, fundraising, and other business purposes without further notice, approval, or compensation.

These materials may be used in, but are not limited to:

Studio websites
Parent Portal
Printed publications and brochures
Newsletters
Social media platforms (including Facebook, Instagram, TikTok, YouTube, X, and similar platforms)
Digital advertisements
Promotional videos
Competition and recital programs
Studio displays
Other current or future media formats

I understand that these materials may be edited, reproduced, published, distributed, displayed, or otherwise used by Straight Up Dance Academy without compensation or ownership rights.

Opt-Out Procedure

If I do not wish for my child's name, photograph, video, likeness, or other identifying information to be used, I understand that it is my responsibility to notify Straight Up Dance Academy in writing by emailing straightupoffice@gmail.com.

My written request must clearly specify whether the restriction applies to:

Photographs
Video recordings
Student name
Audio recordings
All media

I understand that opt-out requests will apply only to future publications and media created after the request has been received and processed. I acknowledge that Straight Up Dance Academy cannot guarantee the removal of materials that have already been printed, published, distributed, shared on social media, included in advertisements, or otherwise publicly released before the opt-out request is received.

If my media preferences change at any time during the dance season, I agree to promptly notify the Studio Director in writing. Verbal requests or requests made to instructors or other staff members will not constitute official notice.

I understand that while Straight Up Dance Academy will make every reasonable effort to honor my written request, the studio is not responsible for photographs, videos, or recordings taken, posted, or shared by audience members, other parents, competition organizers, photographers, videographers, media outlets, or third parties who are not acting on behalf of Straight Up Dance Academy.

By signing this Agreement, I acknowledge that I have read, understand, and agree to the terms of this Photography, Video, and Media Release.
