Liability Waiver, Media Release, and Parent Agreement
I, ____________________________, the parent or legal guardian of the applicant listed above, hereby grant permission for my child to participate in all Integrity Dance Company rehearsals, classes, performances, programs, events, and related activities.
I understand that participation in dance and related activities involves inherent risks, including the risk of personal injury. On behalf of myself, my child, and our heirs, assigns, and representatives, I voluntarily assume all risks associated with participation and agree to release, waive, discharge, indemnify, and hold harmless Integrity Dance Company, its owners, directors, instructors, staff, volunteers, sponsors, agents, officers, and representatives from any and all claims, liabilities, demands, damages, losses, costs, or expenses arising from or related to participation in any Integrity Dance Company activity, whether as a participant or spectator. This release includes responsibility for any medical expenses, insurance deductibles, or related costs that may be incurred.
I grant permission for Integrity Dance Company to photograph and/or record video of my child and members of my family while participating in classes, rehearsals, performances, events, or other program activities. I understand that these images and recordings may be used for promotional, marketing, educational, social media, website, or other program-related purposes without compensation.
I understand that Integrity Dance Company is not responsible for participants before arrival or after dismissal from classes, rehearsals, performances, or events. Parents and guardians are responsible for ensuring timely pickup and supervision outside of scheduled program activities.
Integrity Dance Company reserves the right to dismiss any student, parent, guardian, or guest whose behavior is disruptive, threatening, unsafe, inappropriate, or detrimental to the well-being of others or to the operation of the program. Such dismissal may result in termination of participation privileges without refund.
By signing below, I acknowledge that I have read, understand, and agree to abide by all Integrity Dance Company policies, procedures, tuition obligations, and applicable fees. I understand that failure to comply with financial or program requirements may result in suspension or dismissal from the program.
