**WAIVER OF LIABILITY AND ASSUMPTION OF RISK**
**Ana-Maria’s Dance Academy Inc.**
I, the undersigned, hereby acknowledge that I am voluntarily participating in dance classes and activities at Ana-Maria’s Dance Academy Inc. I understand that participation in dance activities carries inherent risks, including but not limited to the risk of injury. I acknowledge that I am fully aware of these risks and willingly assume all such risks associated with my participation.
In consideration of my participation, I hereby release, waive, and discharge Ana-Maria’s Dance Academy Inc., its owners, employees, instructors, and agents from any and all liability for any injury, loss, or damage to myself or my property that may arise from my participation in dance classes and activities, whether caused by the negligence of the above-mentioned parties or otherwise.
I understand that there are no refunds for any classes or fees paid to Ana-Maria’s Dance Academy Inc. I further acknowledge that if I have any questions or concerns, I will contact the studio office directly rather than attempting to address them with instructors or staff members in person.
By signing this waiver, I confirm that I have read, understood, and agree to the terms and conditions stated above.
I, the undersigned, hereby grant Ana-Maria’s Dance Academy Inc. and its representatives the irrevocable right to take, use, and publish photographs, videos, and other media of my child or myself (collectively referred to as "Media") for promotional purposes, including but not limited to brochures, websites, social media, and other marketing materials.
**Acknowledgment and Consent:**
1. **Consent to Use Media**: I understand that the Media may be used without any further consent from me or my child. I acknowledge that Ana-Maria’s Dance Academy Inc. will own the rights to all Media taken during classes, performances, or events.
2. **No Compensation**: I acknowledge that I will not receive any compensation for the use of the Media and that all rights to the Media are relinquished.
3. **Release of Liability**: I hereby release, waive, and discharge Ana-Maria’s Dance Academy Inc., its owners, employees, instructors, and agents from any liability for any claims, demands, or causes of action that may arise in connection with the use of the Media.
4. **Understanding of Terms**: By signing this waiver, I confirm that I have read and understood the terms and conditions outlined above and that I am giving my consent freely and voluntarily.
5. **Right to Withdraw Consent**: I understand that I may withdraw my consent at any time by providing written notice to Ana-Maria’s Dance Academy Inc. However, I acknowledge that this will not affect any Media already published or distributed.
By signing this waiver, I confirm that I have read, understood, and agree to the terms and conditions stated above.
