All participants and students are required to complete this form prior to participating in any classes or rehearsals at Rohr Studios. If the participant is under 18 years of age, a parent or guardian must also review and sign this form. Admission to classes and/or rehearsals will not be granted without receipt of a properly completed and signed form prior to the scheduled activity.
WAIVER OF LIABILITY:
I acknowledge the inherent risks of physical injury associated with dance, dance training, acro, and other physical activities, and I voluntarily assume those risks. I release Rohr Studios, its owners, affiliated dance studios, event sponsors, employees, and instructors from any liability for injuries or illnesses incurred while attending or participating in classes, rehearsals, workshops, or performances. I agree to indemnify, defend, and hold harmless Rohr Studios and its affiliates from any liabilities, costs, or judgments arising from acts or omissions on my part or on behalf of my child that result in injury or damage to persons or property.
PHYSICAL CONTACT:
I understand that dance instruction often involves physical contact from the instructor to guide and correct movements, techniques, and positioning. This may include touching arms, legs, feet, hips, back, and head for instructional purposes and safety. I acknowledge that such contact is a standard part of dance education. I also understand there may be rare circumstances where an instructor might need to hold or support my or my child's body in private areas in the event of a fall or similar situation.
PROPERTY SECURITY:
I understand it is my responsibility to safeguard my personal belongings while attending classes, rehearsals, workshops, or performances. I release Rohr Studios, its owners, affiliates, sponsors, employees, and instructors from any liability for loss or damage to personal property. I agree to comply with all rules, policies, and regulations established by Rohr Studios.
MEDICAL AUTHORIZATION:
In the event of injury or medical emergency, I authorize Rohr Studios to arrange transportation and medical treatment for myself or my child as needed. All costs associated with emergency services will be my responsibility. In case of an extreme emergency, or if my child is under 18, Rohr Studios will attempt to contact the emergency contacts provided below regarding my condition and whereabouts.
PHOTO/VIDEO RELEASE:
Rohr Studios may use photographs, recordings, and videos taken during classes, performances, or events for instructional, promotional, and advertising purposes across various platforms. If I or my minor child do not consent to such use, I will notify Rohr Studios prior to participation.
RULES AND CONDUCT:
I agree that my students, myself, and my guests will adhere to all rules, policies, and procedures set forth by Rohr Studios, its faculty, and the Studio Handbook.
COVID CONSENT AND SAFETY PROTOCOLS:
Rohr Studios will implement the health and safety procedures outlined below to the best of its ability and reserves the right to update these policies as necessary. I agree to comply with all COVID-19 protocols and participation requirements.
- Students must undergo COVID testing if they or anyone in their household has been exposed to COVID-19 or shows symptoms such as cold, flu, fever, or COVID related symptoms within the past five days. Testing and symptom status will determine attendance via Band.
- Students may not return in person until all household symptoms have subsided for at least 24 hours. Afterward, they may attend with strict masking for 7 days. After 7 days of masking without symptoms, they may return without a mask.
- If symptoms reappear at any point, the process resets, and students must follow the same protocols anew.
General Acknowledgment:
I understand that this waiver supplements previous documentation and that participation is voluntary during the ongoing COVID pandemic. I am aware that COVID can have a long incubation period and that testing limitations may prevent identification of infected individuals. I recognize that travel and in-person attendance can increase infection risk. I agree to hold harmless Rohr Studios, its staff, and affiliates from any claims or liability related to COVID transmission. I acknowledge the inherent risks of being in proximity to others and accept that despite precautions, transmission remains possible. By sending my child to class, I affirm that I have reviewed and understood this consent, and that my child and household members are currently symptom-free. If not, I will refrain from attending.
SHARING OF INFORMATION: Rohr Studios may share a customer's name, contact details, payment information (e.g., credit card number, billing address), and other relevant data with third-party payment processors to facilitate transaction completion. Information is shared solely for the purpose of processing payments for purchased products and/or services.
Third-Party Providers: Rohr Studios collaborates with reputable third-party payment processing services that adhere to industry security standards such as PCI DSS to help protect data.
Data Security: While we take steps to ensure data security, compliance with PCI DSS does not have legal authority, and adherence does not guarantee protection against data breaches. Third-party providers implement security measures including encryption, tokenization, and multi-factor authentication to safeguard your information.
Customer Responsibilities: Customers are responsible for ensuring the accuracy and security of their payment information when submitting it through our platform or the third-party payment system.
Acknowledgement of Risk: Sharing information through online systems involves certain risks, including the potential for data breaches.
Release of Liability: To the fullest extent permitted by law, customers agree to release and hold harmless Rohr Studios from any claims or damages arising from or related to the sharing or use of their information by third-party payment processors under this agreement.
Acknowledgment of Waiver:
By signing this release, I confirm that I have fully read and understood its contents. I acknowledge I am at least 18 years old, competent to sign this agreement, and that I am doing so voluntarily. I confirm there are no health issues that would restrict participation and agree to pay for any medical expenses resulting from injury.
