Liability Waiver 25/26

I acknowledge and agree that participating in any dance class, workshop, rehearsal, or performance carries a risk of physical injury or even death. Consequently, I voluntarily assume all risks and responsibilities for any injuries or accidents that may occur to me or my child during any Dance Clinic classes, rehearsals, performances, or activities.
I hereby exempt, release, and indemnify Dance Clinic, along with its owners, agents, volunteers, assistants, employees, guest artists, faculty members, and/or students, from any and all liability claims, demands, or causes of action for any damage, loss, injury, or death to myself, my children, or our property that may arise from participating in any classes or activities conducted by Dance Clinic.
Furthermore, I voluntarily waive my rights and those of my heirs and assigns to hold Dance Clinic and its associated individuals liable for such damage, loss, injury, or death. I understand the importance of being aware of my physical limitations and agree not to exceed them. If I am signing this waiver on behalf of my children, I confirm that I am their parent or legal guardian and have the authority to waive these rights.
