Media Consent Form

I consent to Creative Dance Industries Pty Ltd photographing and recording my child during the After School Dance Program, including classes, rehearsals, and performances.

Recordings may be used privately and internally, such as sharing performance videos with participating families and for program purposes.

I understand that recordings may also be shown publicly only in school assembly videos or similar school presentations. Creative Dance Industries will not use student media for general advertising or social media without separate written consent.

I acknowledge that I have read and accept Creative Dance Industries’ use of media as outlined above. If I do not wish my child to appear in school assembly video or other limited public presentations, I may opt out at any time by updating the Waivers section of my Studio Pro account.

Opting out applies to future use only and does not affect private/internal recordings or media already created.
